Drainage journal:
21/4/23. Done with a small calibre needle without interventional radiology (IR), but did use a bedside ultrasound. 20ml drained before blockage. The initial thought was that the blockage was due to loculations (pockets of fluid, rather than a single 'bag' of fluid).
9/5/23. Assessment made for a second drain without IR. Bedside ultrasound showed no loculations, but lots of mucin. The attempt was aborted, as the doctor determined that IR was required.
19/5/23. Drain done with 14 French needle with IR. 2.8 litres drained before blockage from mucin.
8/6/23. Drain done with 8.5 French needle with IR. 1.5 litres drained before blockage from mucin.
18/6/23. Drain done via laparoscopy incision. 7.0 litres drained without blockage. The surgeon estimated he got 90% of the ascites out, which is par for the course.
30/6/23. Drain done with 16 French needle with IR. 4.2 litres drained before 90% blockage from mucin.

I’m 32 single mom of one child that doesn’t have cancer or know anyone with it thank god. I’ve somehow found myself watching for your updates and definitely praying for you. What a wonderful thing to ask people for! I used to donate blood fairly regularly, but now, at 74, they won’t accept me unless there is some great catastrophe. That being said, I applaud you for what you are promoting/asking. Blood donations are the most precious gifts we can give. Ever. May everyone’s heart be moved by your plea. ❤

My last comment. My youngest daughter had a complete hysterectomy 2 years ago and her doctor took her appendix. At the time I thought that was a bit strange, but now after watching the linked videos I’m on my knees thanking God her physician was smart enough to know what could happen. You are such a smart and personable man.. if our paths would have crossed in a different lifetime I know for sure we would have been the best of friends.

so the above comment is referring to:
a new technique in Berkeley, CA called SonALAsense that uses acid 5 aminolevulinic and ultrawave sound of echography to treat cancer. It is a non-invasive method that is still in clinical trials, but it has the potential to be a very effective treatment for glioblastoma multiforme (GBM) and other deadly cancers.
SonALAsense works by using MRI-guided focused ultrasound to deliver aminolevulinic acid (ALA) to tumor cells. ALA is a naturally occurring substance that is converted into protoporphyrin IX (PpIX) by tumor cells. PpIX is a photosensitizer that makes tumor cells more sensitive to ultrasound. When the ultrasound is turned on, it causes the PpIX to produce reactive oxygen species (ROS), which kill the tumor cells.
the first clinical trials for SonALAsense have been completed. The Phase 0/1 trial, which was conducted in recurrent high-grade glioma patients, showed that SonALAsense was safe and effective in killing tumor cells. The trial also showed that SonALAsense was able to shrink tumors and improve patient survival.
The Phase 1/2 trial, which is currently ongoing, is testing the effectiveness of SonALAsense in treating newly diagnosed GBM. The trial is still in its early stages, but the results so far have been promising.
IMPORTANT BIT:
SonALAsense is currently only being developed for the treatment of GBM. However, the early results in GBM are promising, and it is possible that SonALAsense could be effective in treating other types of cancer, including abdominal cancer.
The SonALAsense team is currently conducting clinical trials to test the effectiveness of SonALAsense in treating other types of cancer, including pancreatic cancer, ovarian cancer, and gastric cancer. The results of these trials are not yet available, but they could provide valuable information about the potential of SonALAsense to treat other types of cancer.
5-aminolevulinic acid (5-ALA) is a non-protein amino acid that is a precursor in the biosynthesis of tetrapyrroles. In the medical field, it's primarily used as a photosensitizing agent in photodynamic therapy (PDT). During PDT, 5-ALA is metabolized into a substance that makes the cells more sensitive to light. When these sensitized cells are exposed to a specific wavelength of light, they produce a form of oxygen that kills nearby cells, which is beneficial for targeting and destroying cancer cells.
The mention of ultrasound might refer to sonodynamic therapy (SDT), which is somewhat similar to PDT. Instead of light, SDT uses ultrasound to activate the photosensitizing agent. The ultrasound waves can penetrate deeper into the body than light, potentially allowing for treatment of tumors that are not surface-accessible.
it could provide a significant improvement over traditional chemotherapy methods. However, clinical trials and peer-reviewed research would be needed to confirm the efficacy and safety of this approach.
Assuming the technique combines these two therapies (5-ALA and SDT), it could provide a method to target and destroy cancer cells in a non-invasive way, which would be a notable benefit over surgical methods or other invasive treatments. However, the efficacy of this approach would depend on many factors, including the specific type and stage of the cancer, its location in the body, and the patient's overall health status.

Brilliant idea about donating blood! Maybe also consider a charity/organisation you would like to support, that we can donate to. I will have to suss out with my doctor if I am able to give blood. I’m on blood thinning medication for the rest of my life.
I am a regular blood donor and started after my late Mother was diagnosed with Leukemia and did require blood transfusions. I applaud you for coming forward with your appeal for people to donate blood to help others. Paul, you are one of the kindest and most sincere person I have ever encountered and I hope and trust that through this video, if any of your Subscribers are not already blood donors, they will find it in their hearts to do so. ❤

Paul, your idea of donating blood for the reasons you mention is a beautiful one. I've only donated blood three times, and the third time I was politely told not to come back because it dripped out painfully slowly and they were afraid several times that I would faint, so I'm not a good candidate. When I had to have an IV put in one time they stabbed my hand about five times in the ER before they finally gave up, so I can well imagine what you go through with your treatments. I imagine the healthcare system in Australia is also way more compassionate than the one here in the US where Jenny and Sarah live. So many people go into financial ruin or are unable to get care at all after being diagnosed with cancer in this country. I was on Reddit the other day reading someone's story about how they beat cancer but they are now feeling suicidal because they are in extreme debt and have lost their home and car and basically see no way out.
Yeah, I can't comment on the finances of the American system, but I can comment on the Australian system. Australians get cared for. To get free care in any public hospital in Australia I just have to have a valid Medicare card, which every Australian has. The whole thing is entirely free.
However, except for emergency cases, public health care often has a waiting list, and this can range from days to years. This is where the Australian private health system comes in. Basically, if the public system is too slow for you, then you can get much quicker service in the private system but you have to pay for it. The cost of private health can be offset by private health insurance. The insurance comes in different levels of cover, which is reflected in the annual premium.
In my case, I have Medicare (same as every Australian) and mid-level private health insurance cover. So I am fine.
My entire chemo course done through Medicare would have cost me $0. But doing it through a private hospital with my private health insurance is gong to cost me AUD$500 (US$350), but is done way faster. And with cancer, time is life, so I am gladly parting with that $500.
I do feel sorry for those American that get themselves healthy only to be crippled with debt. It seems incredibly unkind. I do hope that over time the American system can become more like the Australian system.

I'd suppose that the public health insurance/care (Medicare card) is mandatorily paid for by the economically "active" (mostly) employed part of the population by contributions (deductions) from their salaries (income), whereas for the "unactive" part (unemployed, children, disabled, retired), it's being paid for by the State (i.e. solidarity) - at least that's how the public health care/insurance systems are working in the European countries, too.
@@matejmarcak2277 Yes, that is essentially correct. The government funds that pay for Medicare in Australia come from the income tax system. If you pay tax in Australia, then you are contributing to the funding of Medicare.
For very high income earners, there is an extra tax called the Medicare Levy Surcharge. You can read about it here:

So in summary, those without an income pay nothing. Those with an income pay for Medicare through their taxes. Those with a very high income pay the standard amount and then a bit more.
Australia is not a pure socialist democracy, but in the case of Medicare, it comes close. And it seems to work better than a lot of other countries, so I'm gonna say I'm sticking with it.

Great idea to donate blood! During my time working in the laboratory one Bad Trauma we used every unit of blood we had before the patient was transferred to Utah. Keeping the worlds blood supply up is so very important. Helps save lives for sure.

What a wonderful idea! I donated from high school until 2 years ago. That was 40 years of donations. I had to stop as I developed blood clots and I am now on a blood thinner. I now use the time I spent donating blood and I eat bacon instead.

Donating blood is wonderful. I donated blood over the course of 40 years of my life. Each pint donated can save 3 lives...i also donated plasma in between...thanx for bring this up in your video.

this means that I won't again be able to give blood, but I'm always paying it forward in kindness when i can

I gave blood for years, they finally told me to stop. My veins are small and the needles they use are bigger. They told me if they ever needed my veins in An emergency I would have too much scar tissue.. granted this was over years and years of donating. I went every 56 days like clock work. I am now 62. So yes it is one of the most important gifts you can give. I wish I still could. I feel fortunate that I was able to help for so many years of my life.. be well Paul. ❤

Donating blood is a wonderful idea. I unfortunately have a ridiculously low hemoglobin, so I can't donate myself but will ask my family to donate. I've had 2 unrelated cancers in the past 7 years, so I understand the need for donations and applaud your request.

Sounds like you have worked hard to plan for and take care of yourself and your life. I’m glad you have the resources you need to care for your cancer and also have made it less stressful for close family and friends if treatments do not help extend your life as long as you wish. I follow Jenny Apple and POG fam also-I wonder if that is what led me to your videos. I have a couple chronic issues at age 60 and the way you cope really inspires me to keep trying to live life rather than, sometimes, wanting death to find me.

Hi Paul! Up until a few years ago, a good friend of mine and I used to donate blood together regularly - it was a fortnightly ritual - when we lived in Canberra for uni... Just shared your video with her - thanks for reminding us that blood donations matter and for prompting us to start again (even though I'm in Perth and she's in Sydney now)!
Please, please do. And think of this when you do. I will be getting more ascites drains that needs more albumin. There is a real possibility that part of your blood will end up in my arm, helping me directly.
We won't ever know for sure, and it doesn't really matter. What matters is that people that need blood and blood products can get them, and it is thanks to donors like you that this is possible. So thank you.

Paul, your channel has just been recommended to me tonight and I'm both thankful to have seen a slice into who you are, but saddened with this news. You're absolutely amazing. Being able to put a smile on with this going on, being rational and analytical with your words shows what a strong and caring person you are.
With regards to donating blood, this is something I've wanted to do before. Unfortunately here in the UK I'm not allowed to do this due to a diagnosis of my own (ankylosing spondylitis) and the selection of medications I take for it. I will find something to do that honours your request, I give you my word.
I hope you're able to take it easy, and be as well as you can. It's been a pleasure to watch a few of your videos tonight, I'll be keeping up to date with your journey, going along with you by your side. You're not alone.

Such a great idea. Personally, I think it will mean more to me to donate blood as sometimes I’ve felt the cash I’ve given never seemed enough. This way, I feel it would be given with love and hope that it could really help someone goi g through their own medical journey. Will be arranging appt this week. I follow POG family and Jenny too. Hey Paul, your story over your last 25 videos has been inspiring to listen to, for you to lay yourself so bare and so honest has been humbling. You have been making me think more and more about the fragility of life and how I need to make the most of it where I can now at the age of 28. You have also inspired me to book a blood donation which is something I haven't previously wanted to do...I don't know, fear of the needles or something, not 100% sure why. However, having listened to you today, it makes me think what an idiot I have been for not wanting to donate and have now booked my first donation in a couple of weeks time here in the UK.
A few videos ago, you talked about how you were blessed to have had prior warning to your death. I thought that was such a profound and selfless thing to think and I had never really thought of it that way. You are such a strong and top bloke and I wish you the best of luck with your journey and hope to see updates for much more time to come ;)

Since I had chemotherapy less than a year ago I’m not sure they would accept me….. I will check with my oncologist and see what she says…..if she says it’s ok I will definitely donate…..👍❤️🤗🙏

A wonderful suggestion, Paul. I have always strongly believed in importance of blood donation and faithfully donated every 2 months,, especially bc I have the "universal" blood type (O negative). In my 60's now, and after 30+ years of blood donation, I developed severe anemia 5 yrs ago. And I now have another health issue requiring monthly plasma-type infusions for life (just had 1st one this week) for immunoglobin deficiencies. Thankfully, others have donated plasma!

I'm an organ donor too. I did some research, and it turns out that even with my condition, there are still parts of my that can be salvaged for donation. It made me happy to find that out. Paul.
Here in the US, blood drives used to be very common and encouraged. But now, I do not see that same effort being made, so I'm sure your suggestion will make a difference. I'm glad to hear that you are in good shape financially. Financial concerns add a layer of angst, especially for those with small children. I'm not eligible to donate blood, but I will do something else (on your behalf). Most towns have cancer associations that accept money for travel expenses, quilts to keep you warm while receiving infusions, wigs, tote bags, etc. I actually started making tote bags for this purpose a few weeks ago. I have three made and will make a few more and send them to a cancer association who gave my daughter a beautiful wig when she needed one. Thanks Paul. You inspired me to get this done.
I’m a regular donator in my community. I donate platelets. Your right, it is a little uncomfortable but I donate for all the reasons you mentioned. Great video.

I have long intended to donate my entire body after I die.
Not interested in a cremation/ grave or anything like that.
Take out all my organs that can be transplanted (at the very least the eyes should be useful), give the rest to medical students to study as a cadaver.
With all that big talk, I must however confess that I tried donating blood only one time and it did not go well. I threw up and was sick for some time after that. That was a few years ago and I intend to give it a try again now.

I just came across your channel and wanted to reach out to you. I can't thank you enough for sharing your story. I lost my dad to cancer 5 years ago. It happened so suddenly, within a month of his diagnosis, and I couldn't even make it to the hospital in time to see him before he passed away. It's been a constant struggle not knowing what he went through in his final days. Was he scared? Did he cry? Did he feel angry? The uncertainty hurts so much. Your videos have given me a glimpse into what it might have been like for him, and I'm so grateful for that. Thank you for your bravery. I hope you still have many long and happy days ahead of you.

I'm not allowed to give blood, tainted you might say. Born in London, now in ireland and having had a transfusion in the past I'm a big red nono flag. However. Having spent all weekend grieving the loss.of my beloved dog and cat sidekicks, and being a nurse. I like to believe I give in other ways, do no harm, do a good deed every day and treat everyone with dignity as you never know what path they are walking x
