The downward force on his left hand when sliding transferred to his shoulder. Since his hand was planted behind his center of gravity the direction of the subluxation is likely to be anterior.
I've had a very similar shoulder sublexation playing hockey years ago after getting hit from behind (pretty much the same impact Shohei took, but against a wall instead of on the ground). Your range of motion comes back quickly, but muscle usage is the problem. You ~technically~ can use your shoulder muscles, but you feel really unstable and it's INSANELY painful. From what I felt recovering from my injury you would have to be a total psychopath to play through that pain within a week of the injury. But luckily Shohei is a bit of a psychopath so we'll probably see him in game 4 hahaha.
I think you are absolutely correct.. I do believe pain tolerance is real and everyone has pain tolerance at different levels. Also you could of had minor tears that healed with time causing you extreme discomfort but MRI is going to show tears as inflammation definitely tomorrow. I do MRI for a living 😊

Ohtani came into the slide heavy, I noticed his hand hit the dirt hard but he looked to not be generating the speed at the end of his run vs. Mets, vs. Yanks to lay his hip down. Slides from either side take practice, left vs. right hip. Aint really about the knee which naturally tucks unless you are very late to slide. Shohei aint fast enough to use a left foot bag hook from the right knee tuck! The defenders take away a left knee tuck and right toe hook so on this Game 2 injury steal Shoheo has attempted an angle off the bag by curving his midsection off the tag but the tag gets his right knee first! Only thing that beats this tag is a 1985 Vince Coleman or Willie McGhee. Ricky Henderson woulda went headfirst. Right buttocks and rt. knee drop to move away from a tag or dbl play. Shohei slides left knee, hard, hip down hard, indicative of a taller, heavier runner. You can defer to his Japan tapes and his evolution of play. We used to have cat like SS & 2B defensive talents whose style elevated post throw to turn dbl plays. Runners just went hard thru the base in attempts to injure the defender or disrupt the dbl play. We are not see that this post season from players, the contracts could be a reason why. In Game 2, we see an excellent throw from Catch, the tag is there because the defender is quick to the bag and waiting on a slow slide from Shohei. I asked Dave Roberts on Twitter/X vs. the Padres and Mets to replace Ohtani at leadoff to protect his legs and left arm. Beyond reg. season play there is no reason to let this star asset go for records like SB or runs scored. He is a pitcher! Kike or Tommy can move into those 1 or 2 slots or Max, who doesnt steal but can get on base via walks or singles. I also told Dave to feign bunts and have his guys bunt and learn to bunt in key situations. Its fundamental and we see a more jazzy Dodger offense since their Padres games. Shohei's workout routines make him less lighter in the arse than what is normal for that 1 spot. He needs plyometrics and 5 mile runs to lose weight.
