  • It happens to most of us in our 40s. We need cheaters or bifocals to see close up. This condition called presbyopia happens even to people who have never worn glasses. And some people don’t ever want to wear glasses.Dr. Repp discusses how monovision is a way to correct presbyopia and can be achieved with contact lenses, LASIK or cataract surgery to eliminate or reduce your need for glasses.

While monovision is not ideal for everyone, I had been wearing contact lenses in a monovision configuration for over 10 years when I was diagnosed with cataracts in both eyes. We decided to replace the lenses with the same configuration and it has been nothing short of miraculous. After wearing some sort of corrective lens since the 4th grade, I am now free of any kind of correction and can see at distance and also read the paper as well as the fine print on a pill bottle. Somehow, my brain picks up the blurred image enough from my near eye to give me depth perception but also ignores it so there is no double vision or halo during night driving. The only change I have had to make is my target shooting which I used to do with both eyes open. Now I have to close my left eye since at arms length I do get a double sight picture.

My situation is vert similar to yours. After wearing a monovision set up with continuous wear successfully for the past 15yrs I had a cataract removed recently and replaced with an implant. I am waiting to have the other eye done soon and have been advised that monovision implants will do well with me. My consultant has told me that I should wear a pair of glasses with a simple prescription to help me see better while night driving. A plain lens for the left eye and a distance lens for the right. Now I am looking forward to the left eye being repaired.

    This is excellent.
I am now back to 20/20, both eyes.
    Thank you.
The DMV allows 20/60 in one eye, and 20/40 in the other eye.

Very good explanation of the mono vision with cataract

You do not explain why the non-dominant eye cannot or should not be set for distance. My understanding is that it is entirely possible for the non-dominant eye to be set for distance with good results. Can you please comment on this?

Thank you, Dr. Repp. I am 79 and have had cataract surgery. Did I understand correctly that if I do not have a problem with distance vision but need glasses for reading, one mono focal contact lens can correct my non-dominant eye for reading and the dominant eye would not need a contact lens?

Some people don't have a dominant eye (mixed ocular dominance). Can they adapt to monovision?

Hi, Dr. Repp responded: Yes, in that case either eye could be set for distance. They should have no more or less success in adapting to monovision than someone with a clearly dominant eye. Hope that helps.

Before my eyesight got worse with cataracts, I had 20/15 vision. Would monovision be similar to 20/15 vision?

You may get to 20/15, but it will be different. Please discuss with your doctor.

Doctor, question please: I have had cataract surgery in my R eye for distance and it has worked giving me better distance vision. Next, I am doing a contact lens trail for short distance in my L eye. If I then go ahead with cataract surgery for near distance in the L eye (which has a more minor cataract compared to the right eye but I prefer to get this done now rather than later in life) and if after surgery I find I can't focus and see the detail on my computer screen for my work, will glasses help in this situation? Also, can I get glasses that will enhance distance vision for when I am driving?
