The elevator without the 4 is set up that way because in Japan, people associate the number 4 with the Yakuza (Japanese Mafia) and thus there will be plenty of situations where they don’t use the number 4. So that is likely a Japanese Elevator. The reason for the association is because if you get into a situation where you are in the Yakuza’s watch list, one of the common punishments is to get one of your fingers chopped off, leaving you with 4 fingers.
I always thought that it was because that the word for "four" sounds similar if not identical to the word for "death" so most places, especially hospitals, avoid using the number 4. Similar to how a lot of other places in the world avoid the number 13.

The Yakuza thing is just one of a few reasons, just like how yours is another one of those reasons. Most superstitions revolving around fear have multiple reasonings to make them more believable. It’s like how people use the Friday The 13th Movies as a reason to fear the number 13 even though the superstition started before then.
