On number 2, those power lines are so many volts that the guys working on them can't even be touching the ground. Those lines in particular need to be tens of thousands to maybe even hundreds of thousands of volts to get the electricity to travel very long distances due to voltage drop. Even if the workers had an insulated ladder on the ground and were wearing insulated clothing they would still get electrocuted because such high voltages will travel through insulated objects with ease to get to ground.
