  • Health care in the US has traditionally been organized based on a mix of incentives shaped by payers, performance standards, and patient attributes-but not by an element with perhaps the greatest potential to accelerate improvement: place. And even as interest in place-based, multisector public health interventions has increased, questions remain about the extent to which they impact the ways in which healthcare-including primary care-is locally organized, delivered, and paid for.
    In this webinar, we will illuminate the power of place to make health care not just better but more equitable and more aligned with public health goals. Expert panelists will describe bold and novel initiatives-like the federal HEROES program, Johns Hopkins’ Neighborhood Nursing model and Los Angeles’ Community Public Health Teams initiative-that reimagine how healthcare can be delivered, paid for, and aligned by place to improve health outcomes within a community.
