Yeah this is one of those shows that makes no sense while you watch it but it explains it all by the end of the show. You get to see why this and that happened and different povs and then you make connection back to older episodes like OOOOH thats why OOOOH. I loved the og version for that.
The flashback scene at 9:21 was in a lot of trouble because they didn't let Rena in the house in the old movie. So I think Keiichi was able to stay in the house because of Rika's advice (I saw it a long time ago myself, so my memory may be vague and wrong). I think we'll also get a decent idea later on why Keiichi was stabbed so badly and survived. I think Higurashi will recover the detailed setting.

Goma give you a clue that some folks detected: THE CLOCK Their confrontation (started when rena sliced his cheek) was about 14 minutes long... I dont think Keichi and rena could have endured all that time fighting, something happened in those 14 minutes. Also apparently the crime scene changed... so maybe it was an studio error or it was intentional. We cannot trust what we got shown in this episode.
Here is the theory. -We know Rena went crazy and it wasnt just Keichi's paranoia because she says stuff about her that he cant possibly know: Her happiness and her father's issue. -Rena saw Rika and Keichi talking and thought they were talking shit/hiding stuff about her. -Rena went to Rika's house and killed the two girls. -She then visited Keichi. -Keichi was also borderline paranoid at that point. -Their fight most likely ended when Keichi pushed her on the table. Here is the difficult part. Why are Keichi's injuries so different from what we saw? -He was fighting all alone and got injured. Mad people thrashing around do that to themselves. Why is nobody talking clearly about Rena? -If you pay attention nobody actually said that Rena died or was killed. They only said she wasnt saved.This could also mean that she was alive but either escaped(unlikely to do that without killing Keichi) or someone "demoned her away". Did Keichi kill Rena? -Maybe. It may be an extreme theory but what if their roles were reversed after she passed out? What if he was the one doing the stabbing? Just like it is unlikely for Keichi to survive 20+ stabs it is highly unlikely for Rena to not pass out after being repeatedly hit to the temple with a hard object.
Everyone keeps saying there is no way to survive that many stabbing but I think its possible, even if highly unlikely. As for a real life example back in 2014 the girl who survived the slenderman stabbing was stabbed about 20ish times. Also it takes a very long time to die from a gut wound, its why its one of the worst ways to go out.

It's a sequel, in one of the first timelines Keiichi killed Rena due to his Hinamizawa Disease which causes paranoia, hallucinations and clawing at ones neck. He smashed her head in with a bat when he imagined one of his friends who had a marker as a needle. He remembers the old timeline. This is a new timeline after the characters defeated fate, for some reasons they've been reset back in time. Which is why Rika reassured Keiichi to trust Rena so his disease wouldn't go out of control....buuuuut Rena has the disease too, so you know how that went LOL. Also, I don't believe the stabbing scene is a hallucination, because there is information in there that Keiichi wouldn't have know about yet. So I think Rena really attacked him here. It has been foreshadowed a lot in past episodes. And Rena has flown off the deep end worst in past timelines. A stabbing? That's a walk in the park. Shes caved someones face in with a pipe, took an entire school hostage. Out of all of her friend, she likes to fight the most.
