    Again, I've obtained a DO. I did my residency at the number 1 hospital in America. I interviewed at multiple top 10 residencies in my specialty. There were no IMGs on the trail or in the residency I graduated from. My credentials have never been questioned. I am an Anesthesiologist.

      That is a silly point. It doesn't matter whether you are a DO or MD after you match. No one actually gives a shit once you are in residency/practicing. That is honestly some premed-type bullshit. Getting to match is where the concern is here. This convo about what MD vs DO means in practice is simply a means of avoiding the actual topic. The merger is absolutely not going to reduce the bias against img applicants. PDs aren't going to magically stop ignoring them.

    I am a DO class of 1994. I am a pediatric anesthesiologist double boarded in pediatric anesthesiology and anesthesiology. I am fellowship trained at CHOP. I am director of pediatric anesthesiology at my NJ hospital. The Chief of Anesthesiology at my hospital is a DO. By the way, the Chief of Emergency Medicine is also a DO. The Chief Of Oncology was also a DO. My hospital is predominantly MD based by the way. What did you say again about a DO never being able to attain a pediatric neuro anesthesiology fellowship? I was also accepted at every anesthesia position I applied for. That was back in 1998. I have been there and done that so I know exactly what I am talking about. I have nothing against IMGs and would never say anything condescending. There is no need for that. I am one of the most requested anesthesiologists for both adult and pediatric cases. Do I have any regrets? No. .

    the issue is that your view is dated... 15 yrs ago DO was less respected and IMG had better odds... now they have merged DO and MD residencies and the IMGs are being phased out. You realize for every program director in the MD world that only takes MD there is a program director at a former DO residency that is now ACGME only taking DOs. Your info is plain wrong, DO>IMG these days! your information is outdated here, sorry bud! plus what are you talkin about, people match ortho from DO schools all the time! you are literally making stuff up!

    I’m a St. George’s grad. I recently finished my residency in general surgery. I just got a great job as an attending general surgeon where I wanted to be (not in the middle of nowhere). Like you said, you have to work hard to make your dream a reality. I worked my ass off to get where I am. I scored 260+ on my steps, developed vitamin D deficiency during my 2 years on the island because I did nothing but study (true story). I’m much happier having MD at the end of my name rather than DO. I don’t regret the decision. It’s like you said… you need to work hard. The shit that made me end up in my initial predicament where I was denied entrance into an allopathic school… I cut the shit, stopped fucking around, buckled down and studied my ass off. And now I’m living my dream. Agree with your video.

    I’m a DO ophthalmologist and am the surgical director of an ACGME program and we do not look at any FMG applications. I dare you to try and find ONE FMG ophthalmologist. Out of my DO ophtho friends, one is a pediatric neuro ophthalmologist who did her fellowship and still works at UCLA. My best friend from mad school is an interventional pain anesthesiologist. Another one of my good friends is an interventional neuro-radiologist. What exactly is so special about a pediatric neuro anesthesiologists. Both are easy fellowships to get after residency, The only conscious choice they made was to do more training once they finished residency. My co-resident is a neuro-ophthalmologist and orbital surgeon who did his fellowship at Wills Eye and is an attending there now. The DO world might have been much different when you graduated. Oh and I did two years of fellowship in cornea refractive surgery and anterior segment reconstruction. Out of the 6 ophthos that take call at my hospital, the two of us that are DO’s are by far the most advanced eye surgeons in the group

      Kyle Linsey I’m not ripping DOs. Never have. Just stating my opinion based on my very extensive experience in our system. I’ve actually said multiple times that DOs are good doctors and there is an unsubstantiated stigma against them.

    I'm a minority in the US. I'm use to people asking what I am. So going the DO route and explaining what it is wouldn't be anything new to me.

    Hey Dr Buck. I'm kinda disappointed you didn't respond to my post. I hate to admit it but I enjoy reading your blog. This is my take. You applied to go to a us md program and didn't get accepted. You went to the Caribbean school, came back, became a board certified surgeon. Only proves one thing....the us md schools obviously past up getting a great candidate. You proved them dead ass wrong. Personally, I'd be pretty damn proud of that. I'd put m.d. IMG on my name tag. But that's me. Take care brother.

    This might have been true in the 1990s but not true today. Being a Carib MD in 2020 generally limits you to IM/FM and at random hospitals that need bodies. DO has a stigma but I"d rather have the stigma and better options for residency match. ATSU/PCOM/MSUCOM/DMUCOM/LECOM/TCOM all have excellent matching capabilities

      There is absolutely no stigma about “DO” lol no patient knows your a DO or Md. there is a much much bigger stigma against CARIB IMG though, just do a quick search for any specialty and you will see far more DO then Img, there are DO’s matching ag traditional ortho and ent, urology programs etc now in fairly large numbers, that rarely happens from any Carib school. George Washington ortho took a DO this past year and so did Michigan CT surgery, Mayo anesthesia, NYU ortho, the list goes on, I can 100% guarantee that none of those programs have even looked at a Carib application let alone interview them… that’s real STIGMA

    Edit: It's really ignorant to say a DO couldn't become a Pediatric Neuro- Anesthesiologist. I could spend a day listing DOs that are pediatric heart surgeons, Neuro spine surgeons, Interventional Radiologists etc......

    Thank you for an honest perspective. I looked at the NPRM data for IMG vs DO and IMG matches more into Neurosurgery, Orthopedic surgery, and Plastic Surgery for the last four years. SGU, Ross, AUC, Saba are all fine schools. Most people don’t want the more specialized surgical fields. Do what you enjoy and in the end you’re still a doctor. The debate is stupid and ignores international practice rights which may matter to students. Everyone is different and we all want different things from our careers and lives.

      On the seats there is an "S" and this is the high performance version of Audi cars, Porsche has their logo embedded on the headrest. Also, this is a four door car and the only porshe with 4 doors is the panamera and this is way too small to be a panamera.

      It is not an S line... That would imply that it is Audi A3/4/5/6/7/8 with the performance pack. It has Recaro seats with a big S. This is an Audi S3. I am a huge Audi nerd. :)

      This man is the best surgeon in Ut. He is one surgeon that has a great bedside manor. In my case he took over when a different surgeon messed up. I love Dr Parker. He is about the only person that help me make it through 6 month of in and out of the hospital. He has had job in many of the best hospitals in the united states.

    My guess is I'm probably the only academic dean who is responding. If your dream is to go to medical school, DO or MD, go - move across the world to go - move to Mars to go. Just go. Now, Buck is right... it's kind of like the poor billionaires club. Yep that is right. There is a hierarchy in the billionaires club. Oprah is poor. Those worth less than 3 billion are poor, compared to those worth 10 billion or more - and they are looked down upon. Buck is talking about the exact same thing. The education world is the same way. Colleges matter. Going to reputable programs matter. Ivy League will always get hired over state colleges on every job on the planet. It's flat wrong - but it exists. Example: Mark Zuckerberg dropped out of where? Bill Gates dropped out of where? You already know the answer - Harvard. Paul Allen, co-founder of Microsoft dropped out of where? Guaranteed you have no idea - Washington State University. Who gives a rats ass about WSU. All that to say, academia discriminates. But still - don't ever let it foil your dream. I did my doctorate at the University of Hawaii - go there... studying on Waikiki??? Oh hell ya...

    My mom is a physician recruiter and will rarely interview Caribbean IMGs. She PREFERS DOs. DOs are nicer, more compassionate, have much better bedside manner, and typically don't have their heads stuck up their own asses. I have met tons of DOs and have only met one who regretted his decision. He wanted to be able to practice anywhere in the world, and right now he can't. That was the only thing he didn't like. A lot of DOs are career changers with a ton more life experience. Those schools accept more career changers because of that experience.

It looks different, but when you are a director at a hospital who recruits physicians for 30 years and sees the difference DOs and MDs make in a hospital, there is a distinct difference in their bedside manner but no difference in their level of care. It doesn't matter what it says behind the name as long as they do the job well. She works with two medical schools directly. A DO school and an MD school. The graduates of each are very different. He DO grads are happier, more like family, and nicer. The MDs are more serious, academic, and impersonal. There is a place for both, but it is obvious that each have their strengths and weaknesses. Actually to be honest, as long as the person interviews well and comes from a good residency the two letters behind the name aren't important.

    Thanks for clarifying! Though I think a DO school with a rep like MSUCOM in a DO friendly part of America (like Michigan) > any Carib school. I mean I could be wrong but the DO rep in the Midwest is improving and awareness of the degree is spreading.       I doubt he has any problem with any DOs on a personal level. He is just giving his anecdotal perspective on being an MD vs DO long term. You can definitely have similar levels of success either way

    Dr Buck in all honesty in this day and age no Dr should think he/she is better than the other based on his/her credentials. MD isnt > than DO. Theres just way more MD schools than DO schools out there. Also, as you may know DOs are trained to look at the patient as a whole and normally DOs have better bedside training which means that the majority of them apply for Internal Med/Primary care. No I'm not a DO or MD yet. Im an undergrad looking to apply for one of these programs. At first I believed that MD was the only good program. I realized that was wrong when I started my undergrad at a school with a great DO program. At the end of the day both Drs are here to help the public not bash one another.

    I don't know if you read my opinion on you going to a Caribbean school. So now you're a successful board certified surgeon. Obviously some us md school passed over an excellent opportunity to have you graduate from their school. Its their loss...not yours.. The fact you didn't accept their opinion, says volumes about your class and character. If fact, knowing this I'd refer every surgery patient in both my practices to you.     Most places will take mostly US MDs, some to a lot of DOs, and a few IMGs. Most places who place emphasis on degree will favor US MDs, take a lot of IMGs and a few if any DOs. It’s just program based. Competitive specialties have shifted in the past 10 years though to be a lot more open to the DO degree. Overall for most programs and specialties competitive or not in 2019 it’s better to be a DO than an IMG on most accounts.

    I have a few thoughts about this video. I do agree with much of what has been said but of course there is a lot left out. First, don't think for a minute Caribbean MD will ever be equal to US MD. Which is why a discussion about Caribbean MD vs US DO is ridiculous. If anything, Caribbean MDs and DOs are placed into a similar category when it comes to bias, although DOs fare better overall. Yes, there are a few schools that take Carib grads over DOs. But, Harvard IM still hates both of us lol! That ain't gonna change. As far as outcomes. I have met many more Carib MDs who have failed to match after multiple YEARS post graduation than I have DOs. I took a look at Ross' 2018 match list. Not one neuro or ortho surg match. My new DO school had multiple neuro and ortho surg matches in the first class. Yes, that's right. The first class. I expect that to improve over the years. When it comes to who harbors the most resentment, I have met as many Carib MDs who harbor resentment as I have DOs. I do think that Carib grads are better at hiding their background/credentials however. So, that MD might be somewhat beneficial. But, as evidenced by this video and the comments below, the chip is still there from all sides. I do think that Caribbean schools are able to recruit a lot of quality Canadian students who were not able to compete for the 10 seats that they have in Canada, and not many DO schools are open to Canadians. If that were to change, I think it would improve DO board scores and match rates. And, lastly, I agree with all said about SDN!

    Dr. Parker, do you realize the number of residencies willing to accept Caribbean students period is relatively small? DO can train pretty much anywhere with the small exception of exceptionally small and competitive residencies like CT integrated. I've met a number of program directors, none would ever say go to a Caribbean school, some even would say no as a last resort.

    Appreciate the followup video (Again, am DO student, hopeful future pediatrician). I’d still argue my point of state MD>DO>Caribbean. Merger has already been happening for the past 3 years. By 2020, ALL programs will have to be switched over, most already are. I don’t think either transitioned program type will immediately accept the other degree in full equivalence but that will fade with time. The super competitive programs are the ones who want research done. You get amazing research, amazing board scores, and they will not care where you came from. In the future it’s going to be more like DMD/DDS. Besides the future will probably be people explaining what a DNP is. Oh wait that’s happening right now. Anyway, still like you more than Dr. Mike #DOsforDrBuck

It's the same standard bullshit education everywhere you go. Some places offer better letters of rec and research opportunities that can enable you to apply for more competitive residencies. That's what makes a place 'prestigious'. DO schools have long underfunded research but that doesn't mean it won't change. Higher chance of changing vs a carribean program thats for sure

    Also if you can't get interviews at US MD schools, you're likely not qualified for a US DO school either! I speak from experience, parent is MD surgeon from top 10 school, sibling at a very competitive DO school, other sibling at SGU.

    My first interview at md medical school went like this. The first doctor to ask me questions was this thoracic surgeon. She spent about 30 minutes busting my balls because I was a pharmacist applying to medical school. She started telling me I couldn't make up my mind what I wanted to do and someone didn't get to be a pharmacist because I took up a seat they could have had and on and on. After about 30minutes of raking my ass over the coals, I got up and walked out.

      Are they allowed to do that during the interview? Grill the interviewees? I’ve heard the interviews being hard but as long as you explain your motivation for medicine and prove her wrong then it shouldn’t be a problem. Kill them with kindness as they say!! :^)

    I understand where you're coming from Dr. Parker. I'm an SGU student and our students match at really good residencies across the board. I looked at where D.O.s from my nearest D.O. school matched and a lot of them ended up filling up spots at the residency sites people use as backups. Thanks for the motivation and for shouting out St. Georges.

    My takeaway was that if you do everything right you get your specialty... well that goes both ways. You're allowed to have an opinion, no one can take that away, but if your opinion is derived from a faulty look at data then your opinion is wrong too.
    Also, you mentioned you might be biased like we are biased because we're applying to md/do schools but saying caribbean sucks is not pushed back upon by anybody other than caribbean students.

    No, you are far more likely to be ranked if you go to a DO school when compared to an MD-Carib school. Period. Dr. Buck's "experience" doesn't matter. The data refutes his thought. I have experienced it. I'm a US-MD who matched into residency.
    Now, that isn't to say that MD-Caribs cannot match. Absolutely not. With sufficiently high board scores it's absolutely possible. And I have nothing but love for the many excellent MD-Carib grads that I have met. But I think Dr. Buck is letting his bias (as he is a Carib grad himself) inform his conclusions here. Does going to a MD school make you inherently better than a DO? No. And vice versa. Every doctor will practice differently and where you went to school means fuck all when it comes to delivering quality care. But if you are trying to give advice (very expensive advice), I strongly believe you should shut the fuck up if you don't know what you are talking about, Buck.

    Hey Dr Buck, what do you know about docs with the MBBS degrees awarded in England, practicing in the US? As far as I know, an MBBS and an MD are equivalent degrees as long as you pass the Steps. Thanks in advance, you're the man!

    I think something he's forgetting to put an emphasis on is the specialty someone wants to get into. If you think you want to become a cardiothoracic surgeon, or a pediatric neuro anesthesiologist, MAYBE IMG is better than DO (most people have no idea what specialty they want to go into until 3rd-4th year of medical school, let alone before medical school). But if you want to go into internal medicine and then do a fellowship and become something like an interventional cardiologist or an oncologist, US DO schools are a much better option in my opinion, because you can easily get into an internal medicine residency from a DO school. And one more thing I disagreed with from the previous video is where Dr. Parker said nobody goes into DO because they want to. I personally know people who got into US MD schools but decided to go to a DO school because they felt like those schools aligned better with their personal beliefs.

    The other md medical school was a new school at Mercer university. I went to pharmacy school at Mercer. I was married and married a 2 year old daughter. I had resigned my job and ready for go. Then I got this call from the school and the accreditation office from my AMA had done their deal and the school didn't meet all the requirements . The lady told me not a good but the school would be delayed by one year. Holy shit....it was a BIG PROBLEM for me. No job and a wife is child to take care of. I call my advisor at Mercer pharmacy school and he told me about a reciprocity program between Georgia and West virginia. I applied and got accepted at WVSOM. I chose it out....excellent school. When I got my packet from the school, I was going to pay in state tuition. Check this out....in state tuition was 600.00 dollars per year. I went to medical school in the USA....all 4 years tuition was 2400.00 dollars.

"Placement" is not the same as matching. 93% of st George or whatever placed into a residency, likely not one they wanted, even more likely some shitty FM residency out in the boonies. What's their MATCH rate, not placement? DOs have a massively higher match rate, AND placement rate. Caribs placement is 93%. DOs average 99.5%.

    M.D.~ DO. IMG = M.D. Successful Orthopedic surgeon here, IMG, U.S. Citizen, M.D. The competitive specialties are competitive for both MD’s and DO’s, however, I can promise you that an IMG with a 240 on the USMLE step 1 will have the same opportunities as any candidate M.D. graduating from a school in the U.S.. DO’s snag competitive specialties, sure, but so do IMG’s. The bottom line is this, it is easier to get acceptance to DO programs and Caribbean programs because of the scores and GPA’s that are deemed acceptable. Of course, you will get a hell of a lot more people getting weeded out of the Caribbean programs because they should not have been there in the first place! Same for the DO’s, many could not cut it and didn’t make it into an M.D. program, so guess what? They will not be obtaining those competitive residencies. There aren’t that many DO schools and to get a Neuro or an Ortho residency, better make sure you are top 5 in your class. Good luck to all.

      What are you smoking? Less than 5 people matched ortho coming out of ALL Carib schools combined the last couple of years. 112 DO’s matched ortho just last year. Lol a 240 Carib step 1 can’t even match anesthesia or OB much less ortho…

    Also, if you are planning to practice outside of US at some point, it is harder to do that with a DO. MD is more widely accepted.

      LOL I don't even know what the hell that means... "frequency of interviewing" then "often, seldom, never"....that is about the furthest thing from a scientific study I've ever seen. Do you know how much variation could be in that? Any bias that could be in that very SUBJECTIVE survey? This is what i'm talking about....you gotta do research first to understand how research should be read. This "survey" is nearly worthless. IF I were to take it serious, then I would point out there is nothing in the "All Specialties Percentage of Programs Citing Each Factor And Mean Importance Rating¹ for Each Factor in Ranking Applicants" that says being a DO or IMG is a factor in their decision making. LOL. It's a joke.

    What it seems most people here are upset with is the underlying selection bias for matriculation at IMG-MD vs DO schools that was not addressed. Although DO schools classically select for stronger candidates, the reasons for which are irrelevant, a given candidate with 3.65 GPA and 30mcat would likely have better future opportunities going to an IMG-MD school based on their preexisting competency. The problem is that a vast majority of IMG-MD students lack this competency. Additionally, there's a slight loophole in the system in that the most competitive DO students have a relatively easier time matching the most competitive specialities (in the DO world) because these positions are not sought by UD-MD candidates. Compare this to the sheer volume of competition to match ortho at an MD program as an allopath.
    Just the two cents of a lowly DO who doesn't feel like taking your allopathic spot.

      Jonathan I would agree with you here. You are correct, a large amount of IMGs are not competent. Which is what I have said before, which accounts for the "lower" match rate. But if you compare the matched DO-IMG in, say competency, I do believe the IMG has the upper hand. And congrats on getting in and becoming a doctor no matter where you are studying:)

      @@drbuckparker Comparing matched DO in the osteopathic world to matched MD-IMG in the allopathic world is comparing apples to oranges; there are way too many confounding variables. A better comparison would be DO vs IMG-MD in matching allopathic residencies (or fellowships), or the previous comparison we mentioned above. In the former, future opportunities, competency, prestige would all be equivalent. It really comes down to vanity and egotism in having the MD after your name. The question applicants should ask themselves is, "Is it worth spending two years in another country away from family, friends, etc for two letters, or do I trust myself to outwork enough students across the spectrum to match where I want?" Success really comes down to intelligence, motivation/work ethic, and the ability to connect and communicate with peers.

    Great video! I am a pre-med looking at Caribbean schools, and it’s encouraging to know that hard work and perseverance can pay off. I pray I can continue to give it my all as I am doing for years to come and one day become an A rated doctor.
