Never heard anybody else say this - that pain pills often create pain, but they most certainly do. The standard opiates like he was taking will do that after a while. First they give you a sensation of wellbeing and relief, but after a while the whole interaction changes - you take them just to feel normal, but they're not really doing much for the pain itself. Then, after a while, you realise that the pain often gets worse when you take them, and eventually, it reaches a point where they are very thing that is causing the pain. Problem is, once you get into long term chronic pain management, it becomes very difficult to tell what is causing what, and many people don't realise what's going on, often upping the dosage to dangerous levels. Most pain is psychological, and these drugs are designed to create long term dependency by ensuring that the perception of pain is ongoing and never really heals. You only realise this once you stop taking them, but of course that is easier said than done, and in some cases it can be dangerous if not done correctly.
Anyone who has suffered shoulder injury will know that even when the rehab in the shoulder you can never really play as hard as you used to. I've never been able to play aggressive music since my rotary cuff repair.
