In terms of whats going on in Varrock, the lore says its a massive city, and the south side are the slums (Hence why everything is in disarray).
Also, there is also what we call scale theory. How the in-game world is scaled down to be practical in-game. When if actually in-universe, it would be much larger in reality (Like Varrock being home to thousands of people).
man that's really well said, and yeah scale theory is so cool. I've seen some stuff talking about that in Skyrim, some modders are even trying to make it real life scale. Pretty cool! Thanks for the comment and insights!
