Psychotic Depression vs Nonpsychotic Depression

  • This video describes the difference between psychotic depression and nonpsychotic depression. If we look at the Diagnostic and Statistical Manual (DSM), there is a diagnosis of major depressive disorder (MDD). Within major depressive disorder, there are severity specifiers. One of those specifiers is MDD with psychotic features. This is referred to as psychotic depression and all the other specifiers of MDD would be nonpsychotic depression.
    About 15% of individuals diagnosed with MDD would qualify as having psychotic depression. In the inpatient population, about 25% of individuals with MDD would have psychotic depression. With psychotic depression there is an elevated risk of the disorder converting into bipolar disorder, particularly if there's an early onset of psychotic depression. Also, relatives of individuals who have psychotic depression have a higher prevalence of bipolar disorder. An individual who has parents with bipolar disorder has an increased risk for psychotic depression, but not for nonpsychotic depression. With psychotic depression, there is an increased depression score overall when compared to nonpsychotic depression.

    I have psychotic depression and its horrible. No voices or hallucinations, but I believed I had weird infections, and I thought someone was stalking me. I thought my parents never loved me or regretted having me, and everyone was making fun of me behind my back.

      skepticalone I had a hard time accepting this diagnosis because I always understood "psychosis" as having hallucinations. With further explanation I understood that thinking that people are always talking about me or looking at me, thinking my family members don't want me around and people are poisoning my food that it finally made sense. I understand everything that you’re going through and this may sound weird but it makes me feel better seeing someone with psychotic depression that doesn’t have hallucinations but extreme paranoia and false perceptions. Thanks for sharing. I hope your symptoms get better.

    The proportion of psychotic depression in older patients with MDD, that you are mentioning here, is shockingly high, doctor. Once more a strong indication that one should never underestimate this disorder. Despite these mental concussions, I enjoyed this video, it's an excellent summary and a much needed closer look at this special mental condition. Thank you very much. ⭐

    i've been dealing with psychotic depression consistently for years. it seems to me like that's more like schizophrenia but i guess i don't understand

      From what I've read, schizophrenia involves hallucinations and things with or without depression. I was wondering that as well because I was starting to wonder if I have schizophrenia. If you look at the criteria between the two, there are a lot of differences

    Psychotic depression does cause memory problems, i wasn't on antidepressants, just to clear that up.

    What’s the relationship between psychotic depression and dissociation? I think I had a psychotic episode during a dissociative episode.

    My brother has always had anxiety..hypochondria..but several years ago he started withdrawing from the family..no more family functions. He thinks everyone hates him and his judging him. Then he started hallucinating seeing aliens and bigfoot everywhere. He would drive to my moms house trying to make it so the aliens could not read his brain..she lives near a army air force base and he thought that could block the aliens from entering his mind. My younger brother saw him this way for the first time and was crying and so my older brother said he hates me now. I tried to explain to him the reality of the situation..but he just could not get it. He was hospitalized and is on medication now but he is still so anxious all the time. TOTALLY dysfunctional just still freaking out a lot. I wish I could help him

    I was diagnosed with MDD with psychotic features and OCD when I was 13. I also had an eating disorder that almost killed me.
    I had a nutritionist who saved my life. "Why are you afraid of this?"
    "I know how food works but for some reason...i just... don't think it works that way for me. "
    I had a delusion that if i ate ANYTHING with fat and eventually ANY food it would turn directly into fat.
    She took the time to explain calories and how cells work. All things i knew and compulsively researched but having a grown up who was licensed in a hospital that I had known for years helped me finally go.
    Bacon . Fuck yes.

    Few months ago my sister was diagnosed with psychosis by a doctor. She suddenly was having panick attacks, dillutional thoughts, fear, little bit of hallucination (it started after having some negative conversation with a male colleague of her). The first two doctors that we saw didn’t say anything like schizophrenia. The medications they prescribed, prevented her from having dillutions and maniac behaviour but her condition wasn’t improving.
    The third doctor we went to, said that she has schizophrenia and his suggested medication improved her condition a lot. In 2 months we had to see the doctor for few times, to adjust the medicines because of some side effects. The doctor increased dosage and added medicines with time.
    But for the last few weeks she is not feeling well. Her fears are coming back again. Specially fear of bathing. She again started to make wrong assumptions about what her friends think about her, mostly negative assumptions. She is worried that she is forgetting things and won't be able to live a normal life like before.
    I'm not sure if she really has schizophrenia or psychotic depression. Should we take opinion from another doctor?

    Well...I'm pretty sure I know what's wrong with me now. Bad news is I also have severe social anxiety, most likely because of paranoia. Depression, and Bipolar runs in my family. That and CPTSD from being sexually abused while very young, and 10 years of literal physical torture by an unwell sibling, not connected to the sexual abuse.

    In my experience typically if the onset of depression is after a major operation, thyroid or heart, its not a psychotic depression just MDD from the individuals i have studied.

    I been diagnosed with severe depression and psychotic symptoms and I'm a alcoholic but I have cut down on the booze alot but still drink and not on any pills just weed and booze but still I cut back on booze nothings better just worse

    I have suffered mdd with psychotic feature first when I was 22 and again when I was 40 sometimes for short periods after I had been binge drinking. Is it possible that I could be bipolar? I was diagnosed with adhd when I was little .

    Is it possible to have Double Depression with psychotic features? Or does it only occur with MDD?

      Double depression is dysthymia (chronic, mild depression) and major depression occurring at the same time. I’m not an expert, I’ve just been diagnosed with double depression, but I would assume that the major depression episode could have psychotic features.
