I think you missed an ability of Giorno. When he punches he can activate an ability that make the person he punched die in infinit amount of time and in an infinit amout of ways in a place between space and time.
That depends on the fact if adaptation can be considered an action. If it is an action then it'll be reversed like everything else. It might be considered a passive ability because Mahoraga has no control over it. Also adaptation works because certain action directly hit Mahoraga like shown in the Gojo fight. So I think that Mahoraga cannot adapt to RTZ because the RTZ itself is a passive ability so it can't be considered an attack towards Mahoraga.
No matter how I look at it GER vs Gojo is a draw. Giorno has no way of hurting Gojo and Gojo just does nothing because all of his actions are reduced to 0
