If he were to ever break out of arkham and go on some sort of killing spree, he'd probably completely lose his sense of empathy, which wouldn't cause as much stress, which wouldn't cause his uncontrollable laughter. As the joker, he'd probably laugh like this a lot. Which is cool. Sounds more like a classic joker laugh. Maybe when he does something REALLY fucked up, he'd laugh uncontrollably, which is even more joker-y. His condition adds so much depth to his character.
