Could you show how to create a Windows form or a browser-based form that would allow a user to create this plate with the four holes, but would let them choose the plate size and thickness, as well as the hole size, and spacing. The form would also prompt them to create a file name, and description, and to pick a material.

That can be done by using a bespoke GUI. Will add this to my list.
