Metabolic Alkalosis (Saline responsive Vs Saline Unresponsive) with hypokalemia and hypochloremia

How to differentiate saline responsive from saline unresponsive metabolic alkalosis with hypokalemia and hypochloremia due to self induced vomiting. Other causes of metabolic alkalosis include primary hyper aldosteronism, Cushing disease.

@@acingmedicine can you pls eloborate the last point pls🥹? Gitelman syndrome cause inc in Cl- in urine so there should be more exchange with HCO3- why this is not happening?
