Thanks for this information. Some nurses use this process to take our medical line organizer product - The Beata Clasp - to their hospital. To keep lines off the floor and easily traceable!!! 😊

Thank you. I just started out as a new risk manager and now I am learning about Safety Events because I am going to start doing them myself.

Great video. Do you think doing a QI project on a near miss due to toxic staff relationships is a great way to address patient safety and the toxic environment?

    Thank you for this video.
    It helped me a lot😊
I have a question. I am wondering when we should interview the patient when we conduct an QI activities.

Typically, quality improvement is more about improving a facility policy or procedure. For instance, finding ways to decrease hospital-acquired infections. In most instances, the patients wouldn't need to be interviewed for that process. However, for a patient safety event or a sentinel event, it's possible that the patient or family/visitors would need to be interviewed if they could provide information about what they observed happening, or what their experience was during the event. I hope that helps! --Meris :)

Dumb question, when would any error in healthcare be acceptable? Wouldn’t “0”% errors or defects always be the standard by default? Could you give an example of when or what area of healthcare would anything but “0”% be acceptable? Thanks!
