E.g. only massive war ships would have a full infirmary, or if we look at non-canon marine forts like G-8, then it’s normal for a ship to retreat to a marine base for treatment if they suffer mass casualties. Marines probably also have more freedom to just show up at random towns and demand healthcare, as long as the town or kingdom falls under world government jurisdiction

Being a vice-admiral definitely does NOT put you in line to be an Admiral. Both new Admirals were outsourced. It seems more like you are promoted until failure in the marines. Then the Admirals are considered a different branch, much like Squad Zero from Bleach.
In the Marines, the rank and file wear the standard military uniform while the high ranking officers can wear whatever they like. As long as they wear the Coat Capes of Justice to be identified as Marines.
Of all of these Japanese names have meanings, there's like a whole different section of one piece that the casual watchers have no clue about. New guy comes in standing there wearing glasses staring at you. You hear his name is something or other, but that in Japanese means guy with glasses who stairs at you, which would be funny if he was his name guy with glasses of stairs at you, but completely missed by people who don't understand that Japanese names have translations into words
