As mentioned, the light goes through the side of his cheek and into his mouth on a wire. It shows this in the "making of" . On the tour the lights thin wire just goes into his mouth instead of through his cheek

This official video is a step back from the live from Madison Square Garden (exactly the one which Suesue reminded, with Till standing high on a tiny platform), both musically and visually. Nevertheless, a great song!

Great reaction from you both as always. Till pierced his cheek for the light apparatus to be fed through rather than have the cable trailing across his cheek. It's a compromise video because the band members spent months arguing over the approach but there's still so much energy in this video. As I recall, Till and Ollie, the bass player, both sore contact lenses for this video.
Apparently they had an idea for a proper music video for this song but it got scrapped and they just went for this simple performance music video, which is not something they generally do for music videos

In this video, Till's light in mouth is added in post editing.
But when he does it live... at 1st he punched a hole in his cheek to put the LED through...but it leaked food/drink and got infected. So instead, he pierced the inside of his mouth to clip the LED inside so it doesn't affect his singing. See Rammlied, live, MSG.
The video of Till on the 32ft high platform is THIS song, live at Madison Square Garden! No harness, just one boot clip for stability.

The only thing added here is Till's lizard tongue. That's one of the director's quirks.

@@SueSueandthewolfman Small scale pyro on stage, flares, and Till had a jacket he'd wear for the song Rammstein...that he'd light on fire, so he just stood there on fire and sang the song! And they were doing the bit with Flake in the boat...
