In order for our teeth to be aligned well and look beautiful, and also fit together as they are supposed to, there is actually a specific mathematical size that each tooth needs to have in order for that to occur. In nature, and in reality, very few people have perfectly sized teeth, so that can be a bit of the problem in seeing maloclussions.
If teeth are very small, sometimes we may have to add tooth structure to accomplish that mathematical size so that the teeth can fit. On the other hand, sometimes teeth are abnormally large, so they may need to be reshaped by removing slight amounts of tooth structure on either side of the tooth, so that they will ultimately fit together.
You can align teeth and make them look very good, but if you haven't established the correct mathematical ratio, or sizes of the teeth, they may never fit well, and be comfortable, and give a good bite. If you don't bite well and you have interferences of teeth, that can contribute to other problems that we see sometimes, in that patients may have facial pain due to overuse of the muscles. Many times, this is called TMJ problems. That can be a result of tooth size discrepancies, and many other reasons for that. That is not a simple cause and effect relationship, but that is part of it.
