  • It's Sunday Night! Welcome! Thanks for watching Probable Cause. The LIVE chat is open during the initial showing and closes once the video has completed.
    This short video contains all the General Aviation fatal accidents from June, plus a bit of new info on the plane crash that I was in on June 17. Heads up! Special cameo appearance by Captain Laura Savino, JetBoss, is in this video as an outside opinion - shot from her house.
    She has a very valid point that I think she articulates very well.
    See her RUclips channel at / @jetbosslaurasavino
    This channel takes a lot of work! I hope that it helps you stay alive!
    I will be off work (my normal flying jobs) for a while because of this, If you are able to offer support, we very much appreciate it. If not, that is OK too!
    I have no sponsors, no Patreon, no Ad Revenue, I pay all my own travel expenses for all speaking engagements.
    But most of all, learn and understand the concepts presented. I work on these videos and travel to locations as I am able. Help me if you can! If you liked this video and my effort, you can donate a very small amount ($1 or $2) direct to Dan for helping to travel make this aviation content possible, and for getting the DC-3 animal air lift mission flight ready. See the web site below for that. Please keep donations to my cause "small" and ONLY participate if I helped you, or if you appreciate the work I did to bring you this special aviation content, or especially if you learned something that can help you stay alive!
    All electronic transactions are safe and secure via the four mobile banking applications listed below. All electronic donation support is received 100% in full, with no fees or transaction percentages lost! Thank you! Dan Gryder
    NEW: the DTSB tax deductible Super Fund page is here!
    Use this for larger donations where tax credit and (possibly) recognition is important: gofund.me/8aa61b2e
    Very small electronic private donations direct to Dan ($1 or $2 ) are very much appreciated.
    You can do that safely via any of these four electronic apps:

    That was my team and I that was trying to save the guy with the water truck. The plane barely missed my loader operator and hit our flex base pile. Not soft at all. It was extremely emotional trying to get to the voice we could hear inside. Plane was upside down and interior debri blocking view, but we did our best flooding the area we thought the voice was. I’m proud of my guys.

      @@ProbableCause-DanGryder we don’t need credit, but I appreciate the gratitude. I have a good friend David Johnson that is a pilot. He made it there pretty quickly and called it a vmc snap roll if I’m wording this correctly.
      Being so close to the airport, I’m going to train my team for responding to an incident like this in the future, which I pray does not happen again. I’m just so thankful that the one man will soon be with his family. I broke down several times that first day not knowing if he was going to be ok.

    I'm not a pilot but I totally get the braking problem. Back in 1959 when I had only about 10 minutes of cycling experience I borrowed a cousin's bicycle. On a steep downhill leading to a tee junction beyond which was a hedge and a beach I discovered the bike had no brakes. I was stopped by the hedge and the gravel. Fortunately there was no traffic on those roads back then. Ever since then I test my bike / motorbike / vehicle brakes before every trip.
    Here's hoping you have a speedy recovery and keep doing what you keep doing: trying to save lives.

      Reminds me... Back in '81, I took a quick ride on a Royal Enfield 250 that my friend had just rebuilt. Just low speed, maybe 15mph. On returning, down the hill towards his closed garage door, JUST getting it to stop before hitting the door!
      He'd cleaned the drum brakes/shoes with paraffin (kerosene). As a 17 year old, finding his way in maintenance, he hadn't realised that was a lubricant and it wouldn't evaporate.
      The pressure on the brake lever was good & braking while going slightly up hill the brakes felt OK (for a vintage motorcycle). So still caught by surprise, how ineffective they were when braking down hill! (AQP!?)
      Not saying this could have occurred (they'd only disconnected the hydraulic line) but some maintenance errors might not be obvious with a low energy brake check.

      I have no real idea what caused Dan's Electra accident, but I have trouble imagining that there wasn't some way out of it when/if the right brake failed after touchdown. Surely that event was not the first time an Electra brake failed during a landing over the years. ???

    • @wangfury
      @wangfury 2 дня назад +1

      That is why there's a tailwheel lock. If used, it keeps the plane rolling straight, as long as the tail is on the ground. That would have prevented the differential braking from pulling the plane off the runway. You can not exit the runway until releasing the tailwheel lock. This accident was caused by a failed or likely unlocked tailwheel. (tailwheel lock expert: Grumman Widgeon, Beech Staggerwing, Hawker Sea Fury).

    • @vgrof2315
      @vgrof2315 2 дня назад

      @@wangfuryCould not argue with you, however there is that period to consider during wheel-landing rollout while the tail wheel is still off the ground.

    • @wangfury
      @wangfury День назад +1

      @vgrof2315 Correct, while the tail is up, the rudder(s) are still effective. When the wing is done flying, the tail is lowered and pinned with full up elevator. At that point, the plane will track straight until lock is released. (usually there's a couple degrees of play in the lock, to allow minor corrections with brakes, to maintain centerline, but you can not turn off the runway until it's unlocked). To prefer flying a swiveling tailwheel airplane without the tailwheel locked for takeoff and landing, is "asking for" exactly what happened. The Lockheed and those people would not have a scratch if the tailwheel lock had been engaged. (Sorry Dan).

    Dan - Big shout out to those who'll run towards the flames!! I got gigged for that in Boy Scouts, & so did my Buddy! We was at Co. Fair w/set-up for Nat'l Guard Mash Type Bell Helicopter demo & rides. Buddies Uncle Pilot, carbs just rebuilt, he made 3 dives at pad, to just pull up @ last second. 4th one, motar no power crashing into pad & stands lighting like Zippo! W/skin in game, we flew over wreckage to Unc trapped as strapped in so tight, whipped out day-old knife guaranteed razor sharp, slashing all straps, Buddy grabbed Unc, I lifted stands just enough to slide Unc outta there, we each hooked an armpit leaving Dodge, but only got 3 leaps, as on fourth those tanks blew, leaving us in jumble, but that 4th power assisted one, just cleared us from flames. We patted out whatever flames were on us, & dragged the hell outta Unc before somethin' else happened! I got gigged in possession of unapproved non-BSA knife @Scout Function, ignoring Asst Scoutmaster's command to come back, & attacking raging fire w/o any fire protection whatsoever! We was stupid, but got Guardsman/Uncle back!! Probable Cause was new shipment of carb overhaul kits, w/couple critical parts not made of Neoprene Rubber!! Couple weeks later, Upon Scoutmaster's return from Yellowstone Vacation. He asked, "Has anyone seen Phil or Erwin?" Little Bobby spoke up sayin', "They were suspended 2 months for stupidity, Sir, and each lost one Rank too",+ Erwin got gigged another month for unapproved knife! The other Scouts laughin' at Bobby, tryin' to get "Brownies" from Asst Scoutmaster, but he added, "My Daddy has movie of it! Oh, Oh! Scoutmaster phoned "Daddy", he brought movie & projector over promptly, & Asst had been right, both of us had been Max Stupid!! We had only survived by fraction of a second leap, & same w/Guardsman. Scoutmaster saw we were spooked, so said, "Men, we're gonna consider this a Training Film, all gigs & demotions cancelled, but where is unapproved knife, it cuts strapping like butter, I just might approve it!" He phoned my Dad, who brought it next meeting, and it was approved, with Scoutmaster sayin', without this sucker, I'd have lost couple of my Boys, Army woulda lost a Guardsman, & we still wouldn't have anything that could slice that strapping!

    • @wangfury

      Your C185 has a steerable tailwheel...the Lockheed does not. If you can't steer it, you lock it, especially for landing, when the rudder loses authority. Brakes can fail, as unfortunately demonstrated here.

    The stall warning started to sound because the pilot trimmed the plane for 60 kts. And then added full throttle without pushing the yoke in order to stay level and gain air speed, almost causing a stall in the process.

    Congratulations on the IFR checkride! It has been many years since my flying days, I was talking with a student pilot the other day and was horrified of his lack of ground school. Several flights in and his instructor was still setting up all navigation for him. Even after a cross country he had no idea how to set a VOR/GPS!! He had zero idea what a VOR even was!!
    ANYWAY...I'll date myself here...wondering if they still train/test on NDB approaches? And here's one....who loved flying a backcourse approach? I despised it! 😅

    Awesome job Dylan!
    Aside on the brake check issue: what changed my choreography in my decent flow was… Cirrus’s checklist. The last item on their descent checklist is… brake pressure check.
    My first thought on classifying the Lockheed accident that Dan was in was mishandled abnormal due to no brake pressure, so already have an AQP number for it? Great job minimizing the danger to the extent possible once on the ground. God bless&much love.

    I have experienced loss of brake pressure on landing (PA28R-201). I didn't panic. I had plenty of runway ahead and walked away. It was a great learning experience and I do a brake check always before landing.

    Brake pressure should part of the landing gear checklist on downwind. For me it's: Gear down, check 3 greens (and manual check if possible like C210), check brake pressure (pump pedals), check handbrake off, landing light on.
