It's both, depending on what you're saying. In English, Shinmyoumaru is her first name, while Sukuna is her last name. Japanese usually say their last name first so it's Sukuna.
Her surname, or family name, is Sukuna. Her given name/actual name is Shinmyoumaru. Wanted to clarify since the first reply is technically right, just sort of misleading. The cultural order or surname/given name doesn't change what the first/last name is and the terms first name/last name themselves are kind of misleading. Its better to use surname/given name for clarity. Also, "English" is irrelevant and something a lot of western animoo fans get confused about when Westernizing something--Not just the name order. Its not specifically English (I.E. Anglicization), but a Romanization (I.E. Roman). That's why its called Romaji (Roma-Ji. Roma is Latin for Rome.). I find this important, because I see a lot of people completely misunderstand things concerning names because they use first/last or assume that its Anglicized.
This is the arrangement that i was looking for!
