Generally speaking if you can see the majority of a model then you can see it. if not then the terrain is considered blocking line of sight. Also Wyldwoods block line of sight if you can't draw a straight line without passing through the wyldwood terrain pieces. Does this help? As far as I could tell I couldn't find a specific ruling in the GHB or the Core Rules, so it is mostly a case by case basis on if the terrain is blocking a model's line of sight.

Not if they can fly. If they don't fly then the same rules apply. If they are fully hidden by a huge mountain they cannot be seen/see, if not then not
