According to the Errata "Rad-phage: A model which loses one or more wounds to an attack with this special rule and survives has its Toughness value reduced by -1 for the rest of the battle." So your command squad were not Toughness 3... a model, not a unit, and you have to lose wounds first, not from the first roll...

Rad grenades😜
It's the errata for the special rule Rad-phage, which is the special rule for rad grenades, rad missiles, Lucifex, Irradiation engine and the rad/irad-cleanser. They all have this special rule and the special rule was errata'd
I think Dans right, rad grenades are their own thing, not using rad phage as a rule. It only counts the turn they charge or are charged as opposed to phage which is a permanent effext
Guys. Thallax are -2 cover saves. And I never used rad phage. Although the irad engines do have it. The archmagos had rad grenades😀
There's a clip missing at 11mins. Basically the frag shells did nothing!
