Arguments, cards, injuries, tackles and hatred. All things dat make the Premier League what it is. Arsenal and City are just the most recent in a long history of beef
If any Arsenal fans wants to find some old games from the 80s, 90s and 2000s go to @DMArsenalArchives channel their collection is crazy. Footballia has a lot of games and all the big ones but not some of the ones against smaller teams

The most underrated rivalries are West Brom v Wolves and Southampton v Portsmouth. Not the best skill-wise but true white-hot hatred. West Ham v Millwall probably the most violent and heated although fairly well-known.

I think your local derby being like fighting a brother analogy is spot on. Another thing may be that many of the regional derbies in Europe aren't on eye level in a sporting sense anymore, whereas rivalries based on success will always be two top teams. That makes the games more intense and always important for your season
