He was a nice guy and nobody likes the fact that he died but his comedy is massively overrated. He was an ok comedian, who had the occasional really good moment but there's a reason he never made it to the top tier of standup. I don't wish death on anybody but if you're in the entertainment industry, boy does it help your career.

He obviously knew. He just chose to smoke anyway. Another joke he has in the same bit is his response when people say he should quit, to which he replies with "but I don't want to" and that's the end of that chat. You can say and I agree as an ex smoker that smoking isn't worth it, but it's his decision and he chose to smoke despite the consequences
