You are absolutely right, there are many such issues with Root canals alone.
Being a sensitive procedure, even a slight inaccuracy or misintepretation of the details of structures involving and surrounding the area of interest in the X-Ray might multiply at further steps of the treatment procedure; and manifest as a bigger problem after completion of the treatment .
In some cases, re root canal is the solution.
I also wanted to ask this:
Could you please upload a re-root canal case, alongwith Ortho cases in the case discussion playlist of your channel.
Regarding the Oral Radiographic interpretation , assesment and treatment planning , this is the most relevant video to be found on RUclips.

In short…manually measure the burr length than compare it with the burr length that ur software claims..difference b/w both lengths is the software error value..always add or subtract that value to or from the value ur software shows….appreciated!👍🏻

It's interesting and amusing to see how differently dentists use x-ray reports while for us they act like a God sent commandant in most cases. 😭😭 And also how precision is required more for y'all and relatively less for us depending upon what was the presenting complaint and what we're expecting. Aoa, sir! I have one question. Would placing the bur in the buccal sulcus make it appear larger than it actually is? It was 21.5 when placed bucally but wouldn't it have been less if placed proximally?
Wasalam Hamza. Doesn’t make a difference because the whole point of this calibration exercise is to tell the software what the exact dimension of the bur is regardless of how big or small it looks on the xray.

Bro RCT k liyea apex locator use karna shuru karo for working length.
You see 2D in xrays so there might be a perforation lingually, which will run along the root anf upto the working length you've measured from xrays ,which will seem like you're in the canal but in reality you're perforating.
