She is pregnant and is singing about how she doesn't recognize herself anymore. It fits into a multilayered story that is much more complicated and is a GREAT musical.
It's those opera notes Rufus Wainwright does that are next level. He's showing a 5 octave range with this performance.....he stole that performance, didn't even know who he was until I heard him sing this.
