THERE IS NO REQUIREMENT IN OUR DISTRICT TO COLLECT A SIGNATURE FROM PARENTS OR STUDENTS. As a teacher, you are just required to "disclose" what you will be teaching in class. Quite honestly it may be a "tradition" that teachers think they need to include that may not be necessary.
