맨손으로해요?
네 압출시 글러브를 착용하고 시술 하시는 곳들이 많지요.
하지만 저희 한의원에서는 글러브 없이 압출이 가능한 경우에는 글러브 없이 압출을 진행 하고 있습니다.
위생적으로 문제가 있을 수 있다는 지적에 대해서 어느정도 공감하고 있고, 저도 가능하다면 모든 경우에서 글러브를 착용하고 싶지만, 치료 과정에서 글러브를 착용하지 않는 것이 더 도움이 되는 것도 많기에 글러브를 착용하지 않고 있습니다.
자세하게 설명 드리자면...
가장 큰 이유는 압출 진행시 작은 씨앗들의 경우 글러브를 착용하고 압출하면 찾기가 어렵기 때문입니다.
압출을 하는 과정에서 여드름 씨앗을 찾을때는 눈으로 보기도 하지만 손으로 만져서 찾는것들도 많습니다. 피부를 살짝 당겨서 늘려보면서 여드름 씨앗을 찾기도 하지만, 그렇게 해도 잘 보이지 않고 손으로 직접 만져야만 찾을 수 있는 것들도 많이 있거든요. 특히나 여드름 씨앗의 방향이나 크기를 가늠할때는 만져보는것이 꼭 필요합니다.
이때 장갑을 끼고 있으면 감각이 무뎌져서 좋은 압출을 하는 것이 어려워 집니다. 그래서 압출시 출혈이 많이 발생할 수 있는 경우가 아니라면 장갑의 사용을 필수적으로 하고 있지는 않습니다.
저희 한의원에서는 가능한한 섬세하고 손상이 덜 가는 방식으로 꼼꼼하게 압출을 해드리는 것을 치료 목표로 삼고 있다보니 여드름의 위치를 파악하고 찾는 것이 중요해서 이런 방식을 사용 합니다.
압출시 위생을 위해서 장갑을 사용해야 한다는 의견을 주시는 분들이 많이 계십니다. 저희도 의료기관인 만큼 압출 과정에서 생길 수 있는 감염이나, 위생관리에 소홀해 질 수 있는 부분은 많이 신경을 쓰고 있습니다.
저희가 압출을 진행하는 과정에서 글러브를 사용하지 않는 대신 하는 방법은
1) 압출 전 후로 꼭 손을 씻는다.
2) 압출 직전에는 손 소독제로 한번 더 소독을 한다.
3) 압출 중간 중간에도 보릭을 이용해서 손과 피부의 청결을 유지할 수 있도록 한다.
입니다.
기구의 소독이나 손톱의 관리 등은 기본적인 것이니 논하지 않구요, 글러브와 관련된 부분만을 정리해서 말씀 드리면,
우선 압출 전 후로 흐르는물에 깨끗이 손을 씻습니다.
압출 과정 중에는 작게라도 출혈이 있을 수 있으니 압출 전후의 손씻기는 필수적입니다.
저희 한의원의 구조상, 환자분이 치료를 받으시는 베드 바로 옆에 손씻는 수전이 있는게 아니다보니 손을 씻는 세면대에서 손을 씻고, 환자분의 피부를 직접 만지는 과정까지, 손이 오염될만한 행동을 하게 되는 부분이 있습니다.
커튼을 연다던지, 조명기구를 킨다던지 하는 부분입니다.
그래서 압출이 들어가기 직전에 다시 한번 손 소독제를 사용합니다. 압출 중간중간에도, 압출을 받으시는 환자분의 피부가 무균상태가 아니기에, 분명히 환자분 피부에서 옮아온 세균으로 손은 다시 세균이 묻을 겁니다. 압출을 진행하는 중에도 소독솜이나 손소독제를 중간중간 사용하여 이런식의 오염도 최소화 하고자 합니다.
글러브를 사용하는 것이 맨손압출보다 무조건 위생적인것도 아닙니다.
일반적으로 다른 치료기관에서 압출시에 사용하는 글러브는 덴탈글러브라고 하는 라텍스나 폴리글러브 입니다. 이 글러브들은 제작시 멸균상태로 제작되지 않습니다. 보통 TV에서 보시는, 수술방에서 사용하는 글러브와는 전혀 다른 글러브로, 수술방에서 사용하는 글러브는 무균상태로 제작되지만 일반적으로 덴탈글러브라고 판매되는 글러브의 경우는 무균상태로 제작되지 않습니다.
그리고 압출을 하게 될때, 환자분이 세안을 하고 치료를 진행 한다고 해도, 환자분의 피부를 완전히 무균상태 수준으로 소독하고 치료를 진행할 수 없기에 치료 과정에서 피부를 계속 만지게 되면서 소독이 되어 있는 무균상태의 장갑이었다고 하더라도 치료 과정중의 오염이 발생하게 됩니다.
그렇기에 글러브를 착용하고 압출을 진행 하는것이 위생이나 소독면에서 맨손압출에 비해 월등하게 뛰어난것이 아닙니다.
장갑의 사용으로 얻을 수 있는 위생상의 이점은 크지 않은데, 사용시 발생하는 치료과정에서의 손해(감각이 무뎌지는)는 확실하기에 저희는 압출치료시 글러브의 사용을 하지 않고 있습니다.
시술시의 글러브 사용은 사실 환자분보다는 시술하는 시술자에게는 이점이 확실하게 있습니다. 환자분의 출혈이나 접촉에 의해서 시술자가 감염이나 기타 오염이 되는 것은 확실하게 막아줄 수 있지요. 시술자로서 얻을 수 있는 이점 또한 양질의 치료를 위해 포기하고 진행 하는 부분인 만큼, 글러브를 사용하지 않는 부분이 조금 거슬리시더라도 좋은 치료를 위해서 양해 부탁 드립니다.
영상을 찍을때만 글러브를 사용할수도 있지만, 압출치료하는 과정을 정직하게 다 보여드리기 위해서 실제 치료하는 상황과 같은 방식으로 촬영이 진행되고 있으니 이점도 양해 부탁드립니다.

16:21 부터 눈 찌푸리면서 봤어요.. 여드름 짜도 나오는 염증도 육안으로 안보이는데 상처에상처를 내시는거 같은데 전문가가보이는건 또 다르시겠죠..?
일부 씨앗이 매우 작은 부분들은 피가 많이 나면 씨앗이 잘 안보이기도 하고, 농이나 씨앗 조각이 조직 안에 남아 잇는 경우에는 일부러 출혈을 유발해서 씻어내는 목적으로 피를 내기도 합니다. 뚜렷하게 씨앗이 보이지 않더라도 염증으로 부어 있는 부분에서는 확인이 잘 안되는 피지나 농이 있을 가능성이 높기에 그걸 좀 씻어내주려고 출혈을 유발하기도 해요.
추가로 바늘로 상처를 내서 출혈을 유발하면 그 부분으로 상처의 회복을 위해 재생을 위한 면역세포들이 모여들기에 염증 반응이 좀 더 빠르게 아물기도 합니다. 물론 그렇게 출혈을 유발한다고 무조건 그렇게 되는건 아니고 염증 반응이 정상적으로 잘 진행될 수 있도록 후처치 ( 저의 경우 한약처방과 약침치료 ) 를 같이 해줍니다.
좁쌀여드름도 혹시 그냥 놔두면 사라지나요?? 물론 염증이 될수도있지만 없어지는경우도있는지 궁금합니다..?? 이마에 좁쌀보면 짜거나 뜯고싶은 충동이 드는데 없어질수있다라는 답변들으면 참을수있을거같아서요...
아 그리고 거울로보면 피부표면으로 튀어나온거같이 보이는데 막상 손으로 만져보면 오돌토돌한게 만져지지 않는 증상은 뭐 어떤경우들인지 궁금합니다

거울로 보면 뭔가 있는거 같은데 만져보면 아무것도 없는거 같은 경우가 있으시다고 하셨는데요, 이런 경우는 주로 건조감이 심해져서 모공각화증처럼 모공안쪽 모낭벽쪽의 피부도 각화가 심한 경우 그러는 경우들이 종종 있습니다.
이런 경우에는 압출을 한다고 짜려고 하면 안됩니다. 정상 피부 조직이라서 압출이 되지도 않고 오히려 피부에 상처만 날수 있거든요.

제가 평소대로 하면 좀 더 신경을 잘 쓸텐데 촬영 중에는 오히려 신경쓸께 너무 많아져서 그런 부분이 소홀한것 같네요. 보통 치료할때는 출혈 부분에 그냥 약솜을 얹어놓는데 촬영 중에는 약솜이 압출 부분을 가리는 경우가 생겨서 바로바로 닦아내고 있습니다.
