You probably have too much insight. At 15 insight, before killing Rom, none of the NPCs worked.. just the tonsil stone guy. But when I had the monster guy near Byrgenwerth eat some of my insight, dropped it down to 13, all of the NPCs worked again.
Go to forbidden woods in the mulino and you find him on the roof just explore that place you will find him ( translate from Italian i can't remember how to say that)

@@blair5475 spoiler alert: with the blood moon you will receive no answer from the doors around the city, people probably go too crazy or die or who knows what happens to them, so you won't find Arianna, the old Lady or the Skeptic man, while Adella and the abhorrent beast guy will simply not spawn anymore, probably dead too

Look at patches and interviews following the release discussing the philosophy of bloodborne for confirmation.
