They used it in the film: "BAT 21" with Gene Hackman and Danny Clover. I don't know about reports from Vietnam war. But reports about Mi-4 were really not bad. In NVA they successfully used it to transport paratroopers.
Hard to do without increasing the size in a piston engine design. They eventually did do an upgrade, S-58/H-34 or Wessex at this end of the pond. It went up to 1500 SHP but took a full redesign of the entire aircraft.

Only a shtload safer. The later PZL version was a lot safer than the Soviet ones.
