    Hey! You are doing amazing work. I have a bit of a situation at hand atm; I am on a Tier 2 visa working with a Bristol based Tech company, whilst my wife is an MS ENT doing her SRship with the same hospital. We are confused what pathway to choose, 1.MRCS -> GMC-> Applying directly for NHS registrar jobs (or SHO? Is it possible to get registrarship directly?)
    2. Mch in Otorhinolaryngology at Edge hill and completing MRCS (I am hoping for her to atleast get the Part A done before the course starts) concurrently.
    Also, can the process of reaching the ST-7/8 (Consultant) be expedited? Or mandatory 5-6 Years of surgical medical training needs to be done before writing the FRCS exam?

      It's impossible to put a measure of this 'difficulty'.
      For some it may be really walk in the park as they quickly develop good professional relationship with the signatory consultant with their work and for some it may prove to be very difficult to achieve.
      So have a look at the latest form and plan and communicate well.

    Thank you for the wonderful video Doctor. As a final year medical student is it possible to achieve the criteria for the 'competitive edge'? For example will my seminars in class, surgical courses attended in my home country, audit during my internship, honours in my university exam still count if I get a certificate signed by my professors? Or only those signed by a UK consultant recognised?

      Yes these things are all useful. It doesn't need to be signed by a UK consultant.

    Does attending atls course provided by local organisation say BCPS, and basic surgical skills course, count to adding points on the portfolio, or is opting for courses offered on the RCS website a better option, because those are quite expensive .

    Could you please outline a similar video on neurosurgery. It’d be of great help knowing that neurosurgery is highly competitive has put me off and I’m considering apply for CST and applying from there for ST3 neurosurgery, is that at all possible ?

    I have a query if u can clear please..... I can not understand the term Restritction to join Core surgical training in terms of experience that u said should be less than 18 months...what does that mean plz?
    Forexample if i have done Posgratduate Residency in General Surgery of 04 years in Pakistan...Then i will counted as over qualified ? Should i give MRCS or not ? if i pass MRCS then i will be eligible or not for surgical training in uk ?

If I completed my residency in General Surgery in my home country and I want to apply to Speciality training, can I apply to ST1 (or higher) having passed the MRCS exam? Would I be eligible?

      Thanks. If you are overqualified for CST, IST wouldn't possibly be any different. You'll have to check the person specifications for IST.
      If you have completed more than 18 months of surgical experience anywhere you'll not be able to apply for CST, but with equivalent competencies signed off you'll be able to apply for ST3 training.

      Don't work in any surgical setting or ensure you work less than 18 months (not including internship).

      That's the criteria. Like having GMC registration and having a CREST form.
      If it wasn’t 'serious' they wouldn’t have make it a criteria.

    I am an IMG who plans to join FY2 and then proceed to apply for CST.
    In order to get the competitive edge and produce evidence for my commitment to surgery, would you suggest I start working(at a non training job) for a year after FY2 and fill in the surgical experience log book?

      Yes as long as you are careful to not go over 18 months of surgical experience.

    But can you please guide how can we fulfil CREST and alternative core competencies while in say Bangladesh Post internship. As opposed to going to UK to do the same, and pros and cons of both?

    with core ST, 18 month rule. is this including housejob/intern year? or does the counting start after that does this include allied surgical specialties too, like orthopaedics, and ENT etc? assuming registered doctor means after completion of internship id imagine the count begins after that...... thanks in advance.

    If an IMG completes MRCS ( both parts), gets full GMC registration, completes CREST and alternate core competencies form in her own country, has a halfway decent portfolio,
    Can she technically apply directly for an ST3 spot?

    He is a medical graduate from Pakistan, has done FCPS (Fellow of College of Physcians and Surgeons Pakistan) and he has also done FRCS from Dublin but he doesn't have a GMC registration.
    Currently he is working as a senior consultant surgeon in Pakistan. How he can work as a surgeon in the UK. ?

    Is it really so very difficult for a Bangladeshi doctor to secure a training post in UK after completing MRCS? I've heard from doctors who are working in NHS that surgical training in Bagladesh is not recognised by NHS.
    I have six months of training in pediatric surgery and nine months of training of urology, general surgery and casualty combined.
    Even though I have decided to go for MRCS but it's quiet disheartening at times.
    Please help me with my confusion.

      Its not any different for you than it would be for any other IMG. You'd need to meet the criteria set out in the application, put together a good portfolio, and apply.

    Hi. i just want to clarify things about being over qualified. Let's say if I have more than 18 months experience in general surgery I can apply for a non training job once I passed the mrcs? And if I passed the mrcs, will I have a full gmc registration? Or do I have to passed PLAB tests and mrcs? Thank you doctor for your response.

      You can apply for specialty training, as the limit is for CST. MRCS or PLAB will get your GMC registration.

    hi, i have a question regarding the entry into core surgical training, if i work in a private hospital in Bangladesh for about 2 years in Orthopaedics dept as plab 2 is continuously being delayed, will it be counted as more than 18 months experience? i am working as a medical officer here. Thanks in advance...

      If you're working in orthopedics, that is a surgical job, and would count as surgical experience.

    1. After doing Post graduation (MS) in General Surgery in India which is a 3 year course, if I obtain MRCS and pass IELTS, can I join ST3 level directly in general surgery?
    2. By doing the above, can I also join some other related specialty such as Surgical Urology at ST3?

      As mentioned in the video, the pathway for you would be the red one, that is, you HAVE to get alternative core competencies signed off before you can apply for ST3 directly.
      Also as I said in the video, it will be unbelievable to get it signed off without ever working in the NHS. That being said, no non-UK training or experience will be automatically recognised as equivalent to CST. It will have to have all the components mentioned in the alternative certificate.
      If you can get alternative core surgical competencies and MRCS completed you can apply in any ST3 level of any surgical specialty.

    I have been working as a medical officer in Urology for 8 months,does it include in my surgical experience and if yes ,then would it fall in the criteria of 18 months surgical experience and make me overqualified after 18 months?TIA

    Is it possible to remain jobless in UK after PLAB 2 passed?
    Or there are high chances to get job?
    I mean is there any saturation of doctors in UK as well like in Pakistan??
    More graduates less jobs ?

      1. You can't 'be in the UK' without a job. (provided you don't have a visa to stay and work otherwise).
      2. There are still thousands of doctor job available in the NHS - but that doesn't mean they will just take anybody and everybody who just passed PLAB 2. You have to succeed in the job application, interview and recruitment process.
      If you can't succeed in that process, you will remain jobless. It will not be the system's fault, it will be your shortcoming in a competitive process.

    I have experience of working as consultant general surgery for 10yrs
    I have done Mrcs Ireland and Mrcsed in 2008 and 2011.
    I got my GMC registration 6 months ago. How should i proceed.

    I wish I came across this while I was working and collected all the needful during the course of time.. Nevertheless, this brings a lot of clarity. I have a question though; The alternative core competency form that I'd need (having more than 2years of non training surgical experience in my home country, India).. Does it need to be signed by UK consultant during the non training job after I'm done with MRCS or can it be signed by a consultant in India under whom I have worked in the past???

      ST3 trauma and orthopaedics is a separate specialty than ST3 general surgery.
