    I've been 2 years seeking medical attention in the best Military hospital in US Soil (6 years with the problem), and NOT a single person ever mentioned something like this. It describes my exact problem and must be this because in Xrays & MRI's there is nothing wrong. Thank's and keep the good job.

      @@ONE-tp8pi Once you treat the ailment, and maintain the treatment, the issue will go away like everything else. Join the military from that point. If I could redo everything, I'd join the military in my twenties and retire at the age I am now in my early forties fit, with a war chest of money saved through the years.

    Thank you so much!! After 8months of pain and numerous visits to doctors and physiotherapists, which didn’t help at all, I’ve found this video started to feel better second day after the training. I’ve been excercising almost every day and after a month I feel almost no pain! Thank you thank you thank you! 🙏

    So an update on my long Piriformis syndrome diagnosed using your simple test, I just completed my first 3 mile walk, that I used to do 4 times a week for the first time in 9 months! No pain at all I can hardly believe it! Your simple reminder to push the knee out rather than letting it fall inwards was all I needed to help correct my posture I cannot thank you enough!

      We’ll it turns out after being able to see a physio in person when restrictions were lifted it’s not so clear if it was piriformis syndrome at all, I had chronic inflammation of the ligaments either side of my tail bone and still have the issue today, I don’t think it can ever be fixed 😩

    It has been a long journey, I have been suffering for 4 years! I went to different doctors: a orthopedist/this moron made to go through a meniscus surgery, it did not help; a kinesiologist taped all body, it helped for a while, but i made me itch. And here you are, I made your treatment-excise for 5 mins, and I do not feel pain and unbalance in my hip, I can walk straight, life literally became brighter and happier!!!

      @@dashhmindset5132 Just do the tennis ball thing and then the clamshell exercise. Keep two tennis balls in an old sock, it is the low cost solution that all weightlifters all over the world use. Tennis or lacrosse ball or a "foam roller".

    Literally no one else has mentioned that there are two different Piriformis syndromes. I've been doing strengthening exercises for months and they are always painful. I now see that I have short Piriformis syndrome, not the long one, so I need to change my approach. Thank you so much.

    This is what I have been looking for 6 months! This is most thorough video to completely understand the symptoms and the corrective action to take. I had been doing the wrong stretches and inadvertently making my condition worse. This has so helped me.

      Same here man. This has been very helpful to me. I've been squatting for a while but I've never experienced this pain until recently. I did the stretches today for short piriformis and my hips have been feeling better today.

      Any feedback after few monts has passed? I've done the tests in tbe video and self diagnosed the short paraformis syndrome on the left side. I also get pain down the inner thigh knee and calf on that side if I stand for a long time.

    Nice one. I'd been diagnosed with Piriformis Syndrome and been given the exercises for short Piriformis. The stretches did nothing for me possibly got worse. Seeing your vid I tried the exercises for long Piriformis. Hey presto - at last some relief! The one that made an instant impression was the standing dip which took away the pain across my butt instantly.

      @@You_Are_Chosen101 honestly I don't remember, I think this video ended up being helpful, but in the end I went to PT and the pain kinda just eventually dissipated. It does flare up every now and then and so I stretch more when that happens which usually helps

    OMG! You literally just saved my life! I’ve been to the chiropractor, doctors appointments and now you! I can’t thank you enough, five minutes of trying to do the one legged squat, my pain went from a 10 to a 2. I’m gonna do this everyday until I’m not falling off the platform.     I was suffering from pain in my left glute area, with tingling sensation down to my left little toe. Doing the short piriformis stretch made it so much better in like 1 min

    Aaron, I cannot thank you enough for this video! I watched countless videos on piriformis syndrome stretching. It wasn't until i came across yours explaining the difference between short and long. Based upon your analysis, I have long piriformis, but was doing stretches for short. I have been in extreme pain for 8 weeks and was making matters worse. I completed day two of your long stretches (squats and clamshells) and am basically pain free! You explanations were exceptional! Keep up the great work and thank you again for getting my health back!

    Thanks for this video! I have that exact pain, I did all those tests and I feel no pain. I have no differences between both of my legs internal or external rotation range of motion... Also I feel this pain only after playing soccer for more than an hour.. I guess it might be that I have the muscle too long? Maybe overusing it, paired with possible poor movement patterns during a soccer match irritates the nerve? I always had a bit of anterior pelvic tilt I guess.. I never had this problem till I pulled my groin really bad 5 months ago.. I am just getting back to playing again and now this.. Again thanks for this video. It's a first one I found that described exactly the type of pain I get..

    Awesome vid!!! Funny thing is, I had the SPS...at the time, didn't know "exactly" how to get to it...would try the stretches but seemed to just miss it...Tried compression/massage but when I got the spot, I would lose it just as quick and get frustrated. Lol Then, one day, I came home super fatigued, sat on my bathtubs ceramic part and BOOM!!! I hit it...jumped up in pain, but a good pain, lol, sat right back down on it and rocked back and forth like a dog wiping it's 🐴 in the grass!!! I had instant relief...not 💯 but definitely noticable.
    Helped a friend of mine out that suffered the same symptoms with a deep tissue, elbow to piriformis massage and she too was amazed of how quick she felt a difference after a failed massage from a guy who made claims he would be able to fix the situation.

    I've got the short piriformis and have for about twenty years. It's worse on my right side, but I do have it a bit on my left side. I find stretching tends to irritate it. For me, walking and very very gentle stretching works best. A little bit of the clam shell exercise is helpful too.
    Mine kind of comes and goes. Gets worse if I have to sit for long hours. I also have gel packs I put in the freezer and put them under my butt when I have a flare up in irritation.

    watching this video through to the end, I am finally able to diagnose that the Long Piriformis syndrome is coming from a lack of turnout engagement in plie while I'm dancing. I always thought I had a short piriformis and that it was overworked and that is so not the case!

    thank you so much for this. I've been squatting, doing glute bridges and all the things that irritated my short piriformis syndrome. I never knew what was wrong with me and even my doctor didn't tell me any of this. I'll keep doing the stretches. thank you so much. Are lunges bad for the short syndrome? Cos I've been doing them for the period of my injury. Thanks again!

      Dee H I too seem to be suffering from short piriformis syndrome which has caused me to take a break from running. I’m still doing my HIIT workouts which include squats, lunges, etc. and I’m wondering if I need to stay away from those as well and just stick with the stretches? Not sure how I ended up with this problem as I already do all these stretches at bedtime and before run. Definitely frustrating! Hope they give us an answer!

    So as a boxer (with my right leg being the one i lean on the most) i had pain for months in my right buttock! It became worse over time. I thought it was because of a weak Glute Medius/Minimus, because i felt it more on the side. However when i abducted my leg to the right, almost no pain ocurred, but when i straightened my leg: it was extremely painful. After a while i started (yesterday) doing stretches for the Piriformis muscle and it was so painful. It felt amost torn & my sleep suffered. I felt the pain towards my knee and even calves. Today, i found out that i do not have a short piriformis syndrome, but a long one. Because my leg rotated very easily up to more than 30/40°.
    Which makes sence, because i overuse the muscle in my orthodox-boxing stance. And when i saw that internally rotating the knee may cause long piriformis syndrome; it all made more sence. Because a punch with the right cross makes the knee bend internally very fast and deep. That obviously took its toll on my piriformis muscle. Went to the shop today and bought some knee bands and started doing exercises immediately to strengthen the muscles around the piriformis and to shorten the piriformis. Hopefully it will help.

      New update: my pain became worse. Especially in the evening. My sleep was really disturbed. The pain is intense. "Uplifting the nerve"-movement does not help, i.e. tilting the head while raising leg while sitting. Thought i had long piriformis syndrome, but the clamshells did not benefit as for now. I even got up at night to do them because of the pain but have gotten no relief.

    I’ve gone deep on RUclips to try and fix some of my issues. I recently had grand mal seizure that changed my body. Brain surgery fixed the seizures. My employer fired me when I was in the hospital and I lost my insurance. So. I’m taking my health care to youtube. I’ve watched 100’s of videos. I was getting turned off by 99% of what I was watching. It’s all amateur “creators” looking for likes and subscribers with the same line. “Pain here? Do this!”
    This was the first video that actually educated me, helped me and added value to my life. Thank you for creating content that is useful and helpful!

    Great video! I've got long piriformis syndrome; I've found that the pain that occurs in the squat or deadlift only occurs at the higher weights. I've done very small increases (5 lb per 1-2 weeks) to my deadlift/squat, but the pain still comes back on with some of these exercises at about 60-70% of my 1RM. It is much more noticeable on the squat. Any ideas on what to do?

      @@trantor5656 buddy the best thing to do is start doing the deep pressure with a tennis ball or foam roller then try to stretch it out bit by bit
      I'm lucky if I can sleep for more than an hour at a time now 😴

    Thanks Doctor. Few weeks ago I asked you about this on Twitter. I was redirected towards your written article on website. Going through your article, I got to know that, actually I was having long syndrome symptoms,but due to lack of knowledge I foam-rolled the spot and actually worsened my condition. Later, after reading your article, I started working on correcting my posture(squat) and strengthening my glutes muscle. And, you won't believe it doctor, I am having better form, lifting more weights. Thanks again doctor, you are doing a great job for betterment of society. NAMASTE 🙏 from India🇮🇳.

    Hola @Squat University. I have a cuestion for you becouse I can't find in bibliography and I am interesting in your opinion please. From 90 degree flexion of the hip, the Piriformis is Internal Rotator. If I want to improve internal rotation of the hip, ¿Is good idea use Airplane exercise with enfasis in internal rotation with flexion of the hip? My cuestion is: ¿ if I improve the strenght of the Piriformis like intrarotator may be improve the general strenght of the piriformis like extrarotator? Thank you very much.

    IMPORTANT!!!! If you have piraformis, WATCH THESE VIDEOS. DO NOT WATCH OTHER VIDEOS BEFORE YOU WATCH THESE. I was just diagnosed last week with piraformis. I watched 6 to 8 other PT videos and they all flatly told you to stretch. So, I stretched. It sent my pain through the roof, forcing me on steroids and 24/7 bed-rest. If I'd seen this video first, I'd have discovered I have "long-form piraformis" and I wouldn't have stretched.

    Your explanation saves my days. Knowing that I'm actually having long piriformis syndrome changed my life. I've done so many exercises (actually for short piriformis syndrome) and taken so much pills for almost a year to make this pajn gone but none of those actually worked.     Can somebody have a long piriformis syndrome on one side and short on the other side?

      It is very unlikely ( but always a very very little chance it is possible). The most common cause of that kind of pain come from your behavior ( sitting on the same glutes, sleeping on the same side, training with a a bad technique). The best way is to look when it is worst and try to understand what mouvement or activity you did .

    Finally, a video explaining everything. I was getting so confused with the stop stretching start stretching videos. Nobody else has said anything about there being 2:forms of this syndrome. Thanks, now I know why I thought I was going to die after I stopped stretching and started the clam shell exercises.

    Dude THANK YOU. Several PTs have told me I have sciatica even though my disks are fine. None of them mentioned this. Nice to have a name for it. Also I def have short syndrome and they gave me exercises for the long syndrome. Clam shells do notttt feel good. It all makes sense now!!!

    I spent so much time and money (with decent insurance) trying to fix short piriformis. I saw a two MDs, a trip to a sports clinic, three PTs and two Chiropractors. Only the 2nd Chiro (a holistic one) correctly diagnosed and fixed the pain (after 2 weeks of visits) by simply pressing on it for two minutes. After that, I sat on a lacrosse ball at the first sign of pain. No pain after 2 weeks. Saved me a trip to the spinal clinic and from botox shots or surgery. The US healthcare system needs a thorough roots and branches shake up.

    Hi Thanks Doc...So informative and through this video I think I have been able to get to the root cause! Long syndrome symptoms due to internal rotation on the follow through of my golf swing of the right hip.? Would you agree and would tight hips aggravate the problem ?

    Excellent video. I think I have the short Piriformis Syndrome since it hurts when I sit down, especially on the couch, or after I have been lying down. It hurts worse in the morning.
    How often can or should one do these exercises and rolling on the lacrosse ball?

    I just wanted to say thank you for helping me diagnose the type of pain and treatment for my long piriformis syndrome! I've been experiencing increased pain this past month and was starting to feel helpless until i found your video. The way you broke things down from diagnosis to specific treatments was very understandable and straightforward! Literally felt almost instant relief after doing only the first exercise!     Watching this at work and for the first time in over a year of experiencing pain in the hip I now understand what type of piriformis syndrome I have and how to go about treating it. I also think that I have lower back related sciatica as well because decompression exercises help me, is this common or a rare condition to have back and piriformis sciatic pain?     I developed UPPER glute pain at the bottom of a squat. Initially, I felt a sharp electric pain, then tingling, then loss of sensation to my legs for a few seconds. 2 weeks later, pain persists. Movement actually makes the glute feel better. As soon as I sit, the pain returns

      @@sackman56 within minutes after I felt the difference. These days I roll on the ball in the effected area before starting my leg training. It's almost gone with jz a few week of doing this.

    Hello doctor, I really need help with something!! I live in Egypt and doctors here arent that serious, they take easy ways and solutions. I have a small L2 L3 protrusion, but I dont feel any pain in the spine nor did any thing wrong but its there and I think its from like over 10 years, but thats not the problem, problem is I have a tingle in my left foot and pointer, middle finger when I only sit!! so I have a question does l2- l3 herniated discs cause sciatica tingling feeling, I dont even have pain?

    I am going to assume I have short piriformis syndrome due to SITTING all day driving a truck. I have been chasing relief from random episodes of discomfort for some time now. However, I feel I need to build my glutes to aid in increasing my metabolism and burn mega calories. My chiropractor has only provided temporary relief and believes I am suffering from its close cousin known as Sciatica.

    I have scoliosis and I am a barber. I love my career but man ol man its hard on my back hips and legs. Today I wanted to cry over this muscle after 10hrs of being on my feet. I think what agitates my muscle is me trying balance myself all day long.

    I’ve had back trouble for years but now Doc sent me to PT and they say I have piriformis syndrome. PT says I’am complicated and I didn’t understand why until I seen your video. It explains so much. I think I have short piriformis but I can’t workout anymore. Any type of squat, lunge, hip bridge, or leg workout makes me miserable for a week. Two months into PT which is basically stretching and it has improved but still unable to work out. Will it ever get completely better?

    For me, it’s not pain at all, but there’s this annoying feeling in my right hip. It’s so irritating, it’s there ever so slightly but not when I’m not conscious about it.

    People don't use a bicycle unless you're in the lightweight category. The bicycle seat devastates the sciatic nerve and the piriformis. Pain radiating to the leg and to the lower back of the spine appears as a disc problem, but the epicenter of the problem is in the pear-shaped muscle that strangles the sciatic nerve. Long distances and seat back pressure will cause severe sciatic nerve pain with consequences.

    This video helped me a lot with trying to solve the issue of a different kind of injury. Every time I squat down, one of the tendons behind my knee (I think its a tendon) keeps popping out, right at a 90-degree angle. Would anyone know what this could be and how I can fix it?

    In past, Literally had several small, hard knots in this muscle. 😩
    Knots gone, but the middle area is always very painful & yes, will shoot down leg & if really bad, it’ll irritate inner thigh at the groin.
    Seems I have the Long P. Syndrome.

    Have been out of action and unable to run since April last year and now I know what the problem is. Physio has definitely been giving me the wrong exercises, making it if not worse, at least keeping me at the same level of injury - since June last year.... Here goes......

    I can’t thank you enough for helping me find the right moves to aid this agony I’ve been saddled with and worsened after being a first responder with the first victim on 9/11 hit me and a strange infection through the left sciatic nerve, glutes surgery (23” long and filetted) that after I got hit hard four years ago as a bodybuilder lost 46 lbs in a month of only muscle and couldn’t breathe stopped the world for me. Sleeping is soo painful I feel depressed and immobilized for the first time and must fight my way back. Will follow you for if I really want my life I’d better do this for me and get off my large Pity Pot! Thank you so much for your help

    I have a herniated disc and piriormis syndrome - now what? My hips are EXTREMELY tight from road cycling and weightlifting. I already had 3 epidural cortisone shots into my lumbar spine. The 3rd shot had zero effect. Shooting pain starts at the glute and goes all the way down to calf. Help!

    Please my friend : at 9.14 I rest 45mn on this position on a chair ( I painted a canvas) after when I got up my buttock was numb followed by a big sciatica for 15 days. NSAIDs and nothing, it was February except that for a few months, the sciatica only comes back when I sit down, even 5 minutes suddenly since May I am standing or lying down. I don't have a herniated disc on the MRI I can no longer sit without confirming this sciatica. knowing that I often sat with my ankle on my knee when I played on the console, is this a long piriform? help me !

    Been dealing with this for a month now. I’m having difficulty getting on the floor to lay on my back. Pain is on the right side and when I attempt o lay down I get spasms and shooting pain radiating from my hip down to my calf. I would also like to mention that that this injury occurred while deadlifting. I work in an office and I believe sitting all day isn’t helping

    Hello! I have a lengthened piriformis according to these tests, however I have very low external rotation capacity on that same side. I would think the stretching of the piriformis would be easy if it’s lengthened. Could I need a mix of both stretching and strengthening?

    In trying to figure out if someone is having Piriformis Syndrome involvement (short or long), when the pain goes down the leg, can this also cause spasms (super tightening with severe pain) in any of the muscles in the upper leg?
    - Semitendinosus ?
    - Biceps femoris (down to pulling hard on the lateral hamstring tendons??)
    I'm talking pain that feels like a literal red hot knife being dug into the back of the upper leg and being pulled down the entire muscle of the upper leg?
    But if you sit down, like driving a car (position) the pain/burn subsides completely?
    And if things are starting to heal up, the pain is almost gone, but if you walk around for more than a minute or 2, those muscles mentioned about spasm again, you can feel the tightening and pulling down to behind the knee and the pain gets progressively worse as you walk.
    But sit down for a few minutes, the pain now subsides.... rinse and repeat?
    Does that sound like something PS can cause?
    Or could this be a pinched nerve elsewhere (or sciatic nerve, somewhere along its length?)

    My piriformis isn’t tight (directly from the hip, definitely not back).. it seems that stretching is doing the opposite, pain continues to radiate down hamstring. Temporary relief so far has been from rolling on a lacrosse ball and abducting.
    Squat day’s coming up, with normal stance width, should I be able to continue my strength training? Again, not experiencing tightened piriformis.
    ps laying on my side with the sciatica flares it up. Sitting on a pc chair also flares it up. Walking doesn’t cause flare up.

    feels like my hip joint starts hurting on one side, only when i go heavy on legpress machine and heavy on conventional deadlift. Is this piriformis syndrome? other exercises like sumo deadlift, lowbar squat ar fine even when doing max weights.

    Hey, I’ve went through the tests and it was pretty clear that my results were short piriformis. After rolling around on a lacrosse ball for a couple minutes I felt much better in that area. However, I felt a lil something on the back of my hamstring when attempting a half hearted a skip. It didn’t affect my ability to move but I felt a touch there that was similar to what I was feeling in my hip - just with no pain. Is it possible that my piriformis and another part of my body are both being affected? If so what would you suggest to combat this other factor??
    Lastly, what sort of cardio if any would you recommend while managing this?

    Incredible video. I saw five regular therapists and they were all having me do exercises you were suggesting for long piriformis syndrome. I finally saw a pelvic therapist and they had me doing stretches for the short piriformis syndrome which resolved my issue.

    I have pain in the anus while deadlifting/squatting.
    It happens when I use the glutes more in a squat and when doing any kind of hinging. The pain is on the right side and I do have a shorter leg on said side (1cm difference). Could this be piriformis syndrome?
    I could really use some help since the two physical therapists I went to ignored this pain entirely since they just said "stop lifting lol"...

    AAron thankyou ,i have watched your video three times now to get it in my head. Christmas time I slipped on the ice and dropped heavily on my left hip. It was really sore but slowly the pain eased a little an the bruising decreased. About a month later the pain started mainly down my left leg from my hip but also to a lesser extent my right side. Both legs are now numb down to my feet and I have difficulty walking. Having done your tests it would seem I have short Piriformis so is it possible to have Piriformis on both sides. Ps My doctor has been treating me for Low vitamin D for three months ?

    I’ve been dealing with, what I believe is, sciatica/piriformis syndrome. It’s have pain running down my left leg when I try to sit down into the couch and what relieves it is doing a modified pigeon stretch on my non pain side. Any advice would be appreciated

    Prepping for a powerlifting meet.
    My piriformis caused an issue a few years back. But i did physio etc and it healed.
    Its acting up again, but i dont want to stop training for the meet.
    Any idea what to do?
    I added glute thrusts to strengthen my glutes so that they take load off the piriformis.
    The pain is reduced but still nagging.

    I have serious back issues and though it was time to go under the knife. I had a feeling it was my sciatica nerve, so I’ve been ball 😮rolling with cross leg stretches but it never goes away unless I just get out of bed. I have confined that it is this issue. I was rolling my left glute with a bigger head ball but switch to the spikes precision ball and just sat on the painful area. I’m dude I am sweating it hurts so bad but slowly it’s releasing. Thanks for the help buddy; been a huge fan of squat university. Life saver!
