I had steven johnson syndrome back when it was called steven johnson skin disease. I was 12 and it was still fairly new, it came about from an allergic reaction I had to some meds I was taking. The way it was explained to me was that basically your body thinks you have a chemical burn and so it sloughs off a few layers of skin. I had blisters EVERYWHERE, and my fingernails and toenails died and fell off (they haven't been the same since). Lasted about a month. I remember it was October because my mom draped me in a vampire cloak and set a bowl of candy outside of my door, told all the doctors and nurses that when they came in they had to grab some and give it too me as if to trick or treat lol. She was a good mom.

The dermatologist I did my senior rotation under was excellent. He was clinically and academically excellent. Yes, I know, it's a lifestyle specialty. But it wasn't for him. I learned more in an hour from him than I did in a week from most other attendings. An excellent dermatologist is an impressive clinician, and they will come when called. I just wish there were more like that. The first patient I saw die was an 8 year old with Stevens Johnson. In the several weeks he was in the hospital his parents never visited, until the last day when his mother showed up. The chief of pediatrics threw her off the unit. Child neglect. At that point the child was within inches of dying, unconscious, and the nurses did more to comfort him than his mother did in the 8 years of his life.

When I was in surgery, my unit cheif had a habit of admitting every case he saw, then not discharging them weeks after surgery,
So we had patients all over the hospital, especially skin ward because it's always empty if not for us,
So once in a while, there'd be a dermat case in the ward, we'd all scratch our head because we don't recognise the patient, why is he here? Who admitted them? Then we'd see dermat people coming for rounds and suddenly we realise it's their ward afterall...

    • @DocSkincare
      @DocSkincare 2 года назад +37

@@phs125 Lol...my hospital's dermatology ward is also filled with medicine patients and the same scenario happens with us when we go and take rounds. We ask interns who is this guy? Is he our patient? What complaint was he admitted for? And then we see his file mentioning that he's under dermatology and we wonder if ever his doctors will come and see him. One of my seniors in medicine would ask of any complaints from these derma patients too since his consulting doctor might not come, so he might resolve his problems since he was on rounds anyway🤭🤭.

I don't know what sort of angels worked in that hospital to identify what she had so quickly, but it was a weekend and we heard that same day she went to the hospital of what was affecting her. Truly a miracle that they did what they did, and I have my mom because of her own strong will and the doctor's quick intervention. When my 4 yo had SJS 27 years ago (a reaction to a cephalosporin given for a sinus infection) our pediatrician started him on pedi-prednisone, then immediately contacted our allergist/pulmonologist (both actually had experience with pediatric patients with SJS). For about a week i spoke every few hours with one or the other. We were so fortunate to already have a nebulizer (he also had asthma), a well stocked pharmacy, and amazing doctors. They did consult with the National Organization of Rare Diseases (NORD) and specialists in Boston. He was a kid who had a lot of allergic reactions so hives were pretty normal, but a passing comment by the pediatrician stuck with me and I had him seen as soon as I saw them spreading after Benedryl. He was sick for weeks but is 31 yo now and has had no long lasting effects (or memories about it). For those who work in the medical field - you absolutely DO make a difference. Thank you!!! ♥️

I went to so many ERs and drs to try to get help once. The last one finally called in a dermatologist. They knew right away what it was. I looked like I had 3rd degree burns all over my body. It started with my hands. They burned so bad I stuck them in ice water. They were red as tomatoes. Slowly the rest of my body did the same. I worked at hotels. People were afraid to even touch the pen I had touched. Turns out I had strep throat at one point. I guess not having tonsils I didnt notice. My throat had not been sore. The infection had gotten so bad it was killing my liver. Which in turn was causing my skin to die and peel off. It was one of the most horrific messed up illnesses I have ever had.

SJS survivor here. My experience sucked. I was prescribed Bactrim for a sinus infection. After 9 of the 14 days of the script I developed a severe rash over about 70% of my body with the feeling of bugs crawling under my skin. I attempted an E.R. visit 2x in one week and was turned away. The 3rd attempt proved fruitful as a resident quickly identified my condition and sent me via ambulance to another hospital 2hrs away that had a burn unit.
By this time I'd lost 40lbs and my skin started peeling. My eyes started to turn yellow and the dr had serious concerns of possible jaundice setting in.
After wrapping my body in bandages, pumping me full of fluids, antibiotics, steroids, anti-inflammatories etc for a week, I was finally able to go home.
Had a dermatologist consult ever been considered, I would've faired better and would have been treated sooner. Luckily I only lost my skin and damaged some nerve endings... and not my vision and all the other accompaniments of SJS.

I remember caring for a teen patient with SJS still as a student Nurse. Dressing his wounds....., was one of the most terrifying experiences I've come across. The worse part was he had his skin sloughing off ALL OVER HIS BODY.
The only place he wasn't sloughing off were his palms, soles, back of thighs and bum, and to that I said he was lucky. It was like he was thrown into a fire, and lost his skin almost everywhere.
He could hold pain when dressing other parts, but when dressing his private parts, it was excruciating for him, and my heart broke everytime. His scalp, eyelash, ears, face, arms legs, feet, toes, head... everywhere was affected.
Was so happy when he got discharged, and he was looking so much better. He looked like he had vitiligo but no hair and was better.
Another patient I saw, was a little girl. Around... 5-6ish, same as a student Nurse, admitted into Cher. It wasn't as bad, just had her skin slouching of and necrotised and some spots. Be cause of the location, she couldn't make it, and I was heart broken.
Both cases were reactions to medications. Its a really severe condition.

My ears started to get ungodly super itchy for no reason and for a long time we couldn't figure out what it was. My doc and me mum couldn't really figure it out, although we suspected it was psoriasis. We told our family doc that a couple of times but he kinda brushed it off. He gave me antibiotic cream, and a different cream with steroids in it. None of them really worked although the steroid cream helped a little. It wasn't until our second visit or so when we happened to mention the magic words "My grandma had it"/"we have family history" that the doc even entertained the idea of it. Set us up with a dermatologist and we were at that appointment for like .2 seconds. We saw the derm, told him we suspected it was psoriasis due to our family history(now that we'd learned the trick) and he took one look at the inside of my ears and went "Yep! That looks like psoriasis alright!" Sent us off with a prescription for my ears and also my scalp as apparently it's common to have an itchy scalp as well. Specialty Doctors aren't just specialty for a reason. Thanks dermatology! Now if only I kept up with my med routine...
