I have not done it in heels personally (I don’t wear heels often especially as of late) but yes, I have seen it done that way. You place the back of the heel, or Achilles area on the pole versus the bottom of the foot if wearing heels.
The foot grip isn't doing nearly as much as the inner thigh grip so having sweaty feet shouldn't impact this as much as it would in something like a cupid.

Absolutely! It is a lot of back engagement similar to what you would use pushing up into a cobra position when lying on the floor.
