    TRANSCRIPT: Hi, I'm Steve Hodges, Associate Professor of Pediatric Urology at Wake Forest University School of Medicine. I want to talk to you about urinary tract infections in children- primarily little girls, because that's who gets most of them.
    Many of you may not be familiar with the urinary tract. Blood goes through your kidneys- you have two of them which produce urine- which flows down to the tubes called the ureters into your bladder, where you store the urine and then evacuate it when you need to.
    Every urinary tract infection in a little girl happens when bacteria from the colon comes out of the colon, travels up into the urethra and bladder and sets up shop. And that's what causes urinary tract infections.
    They can present with varying symptoms- typically urgency and peeing very quickly- dysuria or pain while urinating. Sometimes it can even progress to urinary frequency and sometime fever, chills, nausea, vomiting. And those there are actual kidney infections- which we won't talk about right now- we're going to focus primarily on the bladder infections.
    I want to talk about how urinary tract infections happen and some of the myths about them. So we discussed that every infection happens when bacteria travel into the bladder from the colon.
    So there's only three risk factors that girls can have that cause infections: the primary one is constipation, if you don't let all of the stool out of the colon, that can lead to infections; second, is vulvitis or inflammation of the skin between the rectum and the urethra, that allows bacteria that cause infections to live on the skin more easily and make its way into the bladder; and the third is incomplete or irregular emptying of the bladder, kids hold their urine too long and that is a definite way to lead to infection.
    A good reminder about that is that I very rarely see urinary tract infections in children prior to toilet training. That's because they have less instance of constipation, their moms usually take really good care of their bottoms, and they never hold their urine because they don't have a reason to.
    Some common myths for infection include the way you wipe. In fact it's never been proven that wiping front-to-back or back-to-front has any effect on the incidence of infections. Also, a lot of physicians will tell kids not to take baths and to take showers instead and that really has no bearing on infections- depending on what kind of bath you take. If you're in irritating soaps or bubble baths sometimes it can cause inflammation which, as we mentioned, vulvitis, is a factor in infections.
    But sitting in tubs can be beneficial sometimes especially if you use non-irritating soaps.
    And sometimes if you have irritation on your bottom, just using baking soda or oatmeal or a cup of apple cider vinegar can actually soothe the irritation.
    People often use cranberry juice to help treat infections. Cranberry juice- there's rare data that may help prevent infections, but it's not very strong and it definitely cannot treat infections.
    So the main thing for little girls to do to help prevent infections is number one, treat the constipation; number two, make sure the bottom is clean and dry; and number three is urinate on time. If you do those three things, it's impossible to get an infection.
    If you think your daughter has an infection because she's peeing frequently, peeing very urgently, or having new onset incontinence, or pain with urinating, then you should take her to the physician where they will check a urine study- it's called a urinalysis.
    The child voids into a cup and they look at it under a microscope and there are certain findings on that analysis that will tell them there's an infection.
    If the infection has no fever- just those bladder symptoms we discussed- then it's very simple to treat: you just take some antibiotics and it gets better.
    If the child has fever and nausea/vomiting and flank pain, then that could mean a kidney infection which is much more severe and you may need some imaging studies such as a renal ultrasound or a voiding cystourethrogram to evaluate the child's anatomy to see why they are at risk for these worse infections.
    It's important though, if you have these new bladder symptoms, to see your pediatrician to get this evaluated quickly.

    We found out my daughter has a UTI today, she had back pain for a week then a fever of 103 due to this infection :(

      I’m only 10 and it burns I told my mom she tells me I’m overreacting and doing it for attention. It lasts 8 hours and it hurns

      I had when I was 5 (for 3 long years) and it was very painful, I remember screaming crying most nights because the pain is unbearable. The only reason why I fall asleep is because of exhaustion from screaming and clenching my whole body. Also I can't run around or jump because it will hurt.

    Question. My 3 years old girl. She hurts when she pee but we took her to the doctor and they did urine tests. But they told us that she didn't have infection. She continues to cries when she goes pee. I'm worry what could it be.????

    Hi, I hope my question can be answered. My 2 yo daughter isn't potty trained yet (she knows how to ask, but doesn't want to use the toilet) and she's had ongoing UTI since she was maybe 10 months old. She's had antibiotic treatment after treatment and it's always only good for a few weeks (while on the medicine) and then immediately comes back. Finally they told me she needs to stay on antibiotics forever, even of healthy. But antibiotics aren't really helping and they actually bring her defenses down more each time.
    I'm exhausted trying to find the cause, caring for her hygiene thoroughly, changing her diapers ASAP.. I don't know what else to do. She still has it and gets 3 -5 days of fever every once in a while. I'm honestly done giving her so much antibiotic. I just want to stop this cycle and get her healthy. What can I do???

      That honestly sucks, I have ongoing problems daily and none of my antibiotics are working either, but I may have it gone soon. I'd trying seeing different doctors and find something you haven't tried. When she gets old enough and it's still there she needs to drinks lots of water and unsweetened cranberry juice. It sucks because you can't tell her this at a young age but you need to train your bladder to not have an urge all the time to pee, it sucks but it'll help. Get her pee tested stuff like that. but it's healthy to keep peeing even though it hurts like hell it relishes the bacteria still inside of you. I doubt you haven't tested her because I'm sure that's the first thing you did but make sure she doesn't have bladder cancer. I use ice packs or heating pads. Don't put them right on the vagina or it will get more irritated It dtops the pain but once you take it off it'll burn terribly. Make sure to put them on the abdomen. I know it sucks because she's young and there isn't much you can do, try using creams as well. I hope her health is getting better anything that's similar to uti is overwhelming to handle it the pain is terrible. I really hope everything calms down. I'm sorry for the medical problems 🤦‍♀️😪

      Also don't use wet wipes on the vagina that's how I got my first uti (when I was 7) make sure to wipe front to back and make sure it's not body wash irritating the skin or getting inside of the vagina

      Something similar I had with my daughter ever since she was 5 months old, now she is five and has developed it after 3 years. I think its mostly bc she holds it or bm bacteria traveled bc she can be constipated frequently.

      A friend's of mine was told to put their kid on antibiotics and after 3 years he became so sick she thought he was dying. Long term antibiotics is NOT good for you.

    I had a urine infection at 11 but I didnt tell anyone and I'm 13 nearly 14 now and it doesnt burn anymore. Is this normal, I think I may die.

      You won’t die trust me, just talk to the doctor because UTI can lead to kidney problems. I’m going to get my results back tomorrow to see if I have a UTI

    My daughter is composing of periumblical pain no other symptoms except smelly urine and says ur hurts her tummy when she pees. She is 5. So I'm just guessing what it can be she is still defecating daily. I have appt in few days for her to see pediatrician

      Number one is i decreased the amout of caffeine and sugar.Stayed hydrated. Went to the doctor as needed.Also took Kidney n Bladder supplements.I rarley get it anymore.

    How about my toddler spend too many hours without peeing i take her to the toilet and she tells is not coming but the thing is that she spends mabybe a good 9 to 10 hours withough going not always but some days are worst than other i do offer her plenty of liquid some days she drinks a lot and some days she doesbt should i worry?

      First you'd know that you have uti by having urinalysis if there are wbc's in urine than you'd take antibiotics for 5 to 7 days like noroxin. I'm not telling you about the medications if it travelled to kidneys!

      drink lots of water or cranberry juice if you dont want to treat it by pills ..You can get a prescription but i didnt just took cranberry juice for 2 weeks n it was gone..

    ok so I’ve had a uti a long while ago and treated it at home and I didn’t have to even tell my parents, now I think I have it again and I’ve been drinking lots of water and eating cucumbers it’s less worse then I got it last time, I think it’s mild, will it go away on it’s own and with me drinking lots of water?? please respond I’m only 11 years old and hate this I want to get rid of it, and never get it again 😭

      Hannah Tran Oh! Just drink tons of water and urinate a lotttt everytime you feel the need to go to get the bacteria out and get some cranberry juice, try to make it cranberry with vitamin c in it, that’s the best one and eat cucumbers
