Does the cable really stretch or is it the sleeve compressing?

Yep, they stretch. Especially with lower-quality cables. Brake cables are even more noticeable due to the added pressure put on them. We're talking mm over hundreds of miles, but enough to make a difference.

Should mention that if you dont have a rear barrel adjuster the front one on the shifter works the same way
