Marc- At the age of 37 I was diagnosed with AVN. I had no idea what was happening. I spent 2 months thinking I had pulled a muscle or caused some overuse injury. Then 3 months in physical therapy, which was extremely painful and difficult, before one of the therapists asked why my doctor hadn't ordered x-rays. After 5 excruciating, debilitating months, I found out I had advanced AVN with subchondral collapse. Due to Covid, my surgery would be delayed until May of 2022. I spent nearly a year with this condition, disabled, with intractable pain, and with nothing to do but imagine how my life was over. I had always been active before, biking, hiking, strength training whenever I could. I felt like that part of my life was going to be so diminished that it was the end of those things for me.
And then I found this. It gave me hope. I saw you and your attitude, your obvious fitness, and started watching all of your videos about total hip arthroplasty. I realized my life wasn't over. It was just going to be a little different. I didn't have a disability. I had a challenge. I cannot thank you enough for sharing your story. You made me feel like I wasn't alone. I had my right side done in May, and the left in June. And now, 6 months later, I'm hiking, biking, and getting out the free-weights again. And I was sure to save your videos from all of those months ago when I found them, even though I was barely able to stand then. Because seeing them made me realize that one day, I would be going right along with you. And here I am. Thank you.

That’s great to hear. I have AVN in both hips. They told me to put it off as long as possible. It’s been almost 5 years and I’ve lost so much muscle and the pain is intolerable. I finally schedule my first surgery for the end of the month. And plan to have the second around December.
I know it’s going to be a long trek back to building muscle but I’ve never been more excited to be rid of this. I’m 38 and ready to live the next half of my life pain free.

Be careful with your rehab! Your physiotherapist will give you a booklet which will tell you what you can do and when.
If you overdo it you’re in for pretty severe pain.
I overdid it about 4 mos. after my THR and have really suffered for it.
By ALL means rehab but only what your current state of fitness can handle. Best wishes!

I did both my hips replaced exactly one year ago. Each time I bend down to pickup something from the floor, I literally can't help smiling feeling no pain at all! I understand why people who have had THR surgeries say why I didn't do this mucg earlier. Now I want to keep this metal hips until my final days for at least next 30 years my fear and concern are dislocation when I exercise. I understand I have restrictions now for certain exercises as my hips may pop out if I am not careful and had been looking for this kind of video.
Thank you very much! You really encourage me to keep moving.

I just had a FHR Feb 3, 2022. At 6 weeks out the Surgeon told me I had no restrictions and would have full range of motion. That night I was able to sit cross legged for the first time in my life! Not gonna lie, I cried from joy! Now to try and rebuild all the muscle I've lost. These videos are a true blessing. Going into the surgery, all the PT's I talked to were very skeptical that I would have full range of motion and that I wouldn't really be able to move and exorcise again. Fortunately I found this video a few weeks before my surgery and it really encouraged me. Thank you so much for the information you have shared sir! God bless!

I also appreciate the data you include, such as the video where you mentioned the study about hip precautions (or not). Thank you for making this video, I was diagnosed with severe bilateral osteoarthritis of the hips for no apparent reason, my doctor says that I have a hip replacement criteria, your videos are encouraging, I am only 47 years old but I have been suffering from symptoms for more than 10 years, and The doctors never thought it came from the hips, the diagnosis is misleading at my age, now they say that after 30 you should check your joints

Hello Dr. Mark: Great video! I am just past 12 weeks post-THR on right with posterior approach. My health plan includes 6 post-op sessions with PT, which I've just finished and she was very pleased with my progress. Next week, I'm going to start two weekly classes: A ballet barre class & yoga/meditation class. I will modify both as needed. I have 15 years dance experience, but not able to dance for several years due to hip, back and other injuries from car accident, from which I've slowly been healing (brain injury too). I'll be incorporating your program a few days a week, on my non-class days, and also adding swimming, walking and biking -- all in moderation. My biggest goal is to be able to ride a horse again. I'm doing a lot of slow gentle, hip flexion, glute and hamstring stretches, and practicing climbing onto a padded chair and simulating motion of hips rocking on a horse. Any other suggestions? I still have a bit of sharp pain toward the outside of the hip while doing that. It will be another few weeks before my riding class starts. I'm wondering if when I get on the horse, and there is still that bit of sharp pain, should I try staying on for a few minutes and see if it eases, or get off right away and try again in few weeks. I plan to just start with just one or two 5 min 'sessions' on the horse, per class, then increase the time, till my body adjusts. Due to car accident (2014) I have not been on a horse (or in a dance class) since, so this is a big deal for me. Thanks again for your video!

Dic, really enjoy this video because most patients have no clue as to how to continue strengthening the hips/muscles once outpatient PT is over. At least for me being active andvwhat to continue being active, I'm not sure what to do. I've been following you for a couple months since I'm also here in Los Angeles. I've considered coming to your practice for further training.. I don't want any issues with my body and hips after going through this. I need to get 30 lbs off and continue on my fitness journey. So I appreciate your posts on the things I can do after THR. I'm a double Hippie it hasn't been easy accepting this but it is my new reality.. I teach zumba and was told I can return to teaching but also I need to make sure I strengthen my glutes to hold the implant.. well how what can and can't I do. Appreciate any suggestions

Enjoy the progressive nature of the exercises. It really gives us something to shoot for. One question ; do you recommend we only work the surgical leg or both so that we have equal R/L hip workouts. I’m hoping that your exercises will allow my surgical side to catch up to the opposite, stronger side so I’ve only been doing the one side. Thanks

My physio is great but she’s at a bit of a loss of what to do with me because I am much more physically capable than any patient she has seen before. I'd like to know how soon after the THR should I start this protocol. Incidentally I'm 74 years old and have good upper body strength but the knee and hip issues have caused my lower body strength to be somewhat sketchy. Great Video! Do you have a video for stretching? I am having issues getting into deep stretches : I had a total right hip replacement 15 years ago at age 21. I teach about 10 spin classes per week, weight train, all of the things, but cannot manipulate the 'joint' to alleviate the gluteal knots, and the super tight hip flexor I have.

Thank you for your wonderful advice. How long after hip replacement should these exercises be started.
Is it alright to start them straight away if I feel up to it.
    Much appreciated.

excellent exercises. All of these are also achieved with yoga. My surgeon freaked me out with the threat of dislocation so I don't put my chest on my thighs anymore. I do concentrate on building strength all around my hips, more so than I did before the replacements a year ago. One hip flexor still gives me fits and prevents me from being able to hang from the trapeze and lift my knees up to my chest. Any suggestions?

Thanks for the video. This is a great channel. I will be undergoing hip resurfacing in several months and currently have moderate osteoarthritis. I'd like to modify my current gym workout to prepare for the surgery. Any chance you could suggest pre operative exercises with and without access to gym equipment? I know it's a tremendous amount of work but a video would be great. Specifically I'm interested in knowing if the osteoarthritis is a concern for some exercises. Or if it's better to focus on mobility for the joint and just press on regardless of discomfort. Thanks.

Hi, Hugh. Pretty much all of the exercises here can be done with or without access to a gym. You might be able to get some ideas from the other training video as well.

Hi, I’m 28 I’ve lifted weights my whole life and I just had a total hip replacement 4 weeks ago. The surgery was arthroscopic.
Everything I read says to not extend, hinge, or cross my surgical leg back because it could dislocate. However, my physical therapist thinks I would be safe doing motions such as lunging, single leg dead lifting, and sitting crossed legged position.
In your experience when did you feel safe performing these exercises? It’s not that I don’t feel capable to perform these actions I’m just very nervous to mess anything up.
Anything you can tell me about your first couple weeks after surgery would be amazing and when you felt confident performing certain movements.

I have a question, but would like to detail my hip replacement and where I am at now and then ask your opinion on running and jogging. First, I had a total hip replacement about 2 1/2 years ago and went through the normal rehab over the next few months. By approximately 6 months later, I began playing soccer with my young sons. I did have a setback at one point, as I experienced some type of pinched nerve issues that even limited my normal gait when walking. About 4 months ago, I again returned to a significant increase in exercises, stretching, cardio, squats, some light jogging and a wide variety of total body exercises. What is your experience and advice regarding returning to running? I'd like to do it all, but I'm looking for advice from those that have the life experience on this matter and are further down the road than myself. Thank you
