Adhesive Capsulitis (Frozen Shoulder)
Frozen shoulder is a condition where the shoulder does not move normally. Frozen shoulder is a disorder of the shoulder joint capsule, and it is also known as adhesive capsulitis. In this condition, the patient is unable to move the arm up or turn it to the side (usually there is loss of external rotation of the shoulder). The condition is painful. It occurs due to thickening and tightening of the shoulder capsule and ligaments, especially the coracohumeral ligament. Frozen shoulder can occur by itself or following trauma, even from minor trauma. It can also follow surgery. The process of frozen shoulder usually begins with pain. The pain is severe, especially with movement, and the patient is usually unable to sleep. The patient will be unable to move the shoulder from the pain, and the condition will lead to stiffness of the shoulder. Frozen shoulder may take a long time to resolve, perhaps even several years. The condition of frozen shoulder occurs more in diabetics, especially those who are insulin dependent, and the condition is more severe in diabetics. It can also occur in patients with dupuytren contracture and hypothyroidism. Frozen shoulder tends to occur more in females, especially in their fourth decade of life. Examine the patient’s range of motion both actively and passively. Active range of motion means to ask the patient to move the shoulder. Passive range of motion means the examiner will try to move the shoulder. In adhesive capsulitis, there is decreased range of motion both actively and passively. Try to differentiate the shoulder movement from scapulothoracic movement. Examine the other shoulder because the risk is high for the other shoulder to also become frozen. Exclude dislocation of the shoulder, especially posterior dislocation. With posterior dislocation, the patient will have limited external rotation, the same as adhesive capsulitis. Obtain an axillary view x-ray to differentiate between the two conditions. In posterior dislocation of the shoulder, the humeral head will dislocate posteriorly. In frozen shoulder, the humeral head will remain in its normal location. Not every frozen shoulder is adhesive capsulitis. Please exclude dislocation, diabetes, infection, fractures, and tumors (all these conditions can cause frozen shoulder or limitation of the shoulder range of motion). Treatment is usually complex. Do early aggressive uninterrupted, continuous physiotherapy, pain control, and shoulder injection and manipulation. Surgery is sometimes helpful to release the contractures and adhesions.
I was diagnosed with this 6 months ago. The most painful thing l have gone through in my life. Physical therapy does not help, but l have had to go through it for insurance purposes. I am finally approved for surgery and l pray it works. I have been unable to work, sleep or do daily activities with pain.
How’d surgery go!!!!??

Good video Doctor.
Thank you very much 😊
I was diagnosed with this in my left shoulder. I have Dupuytren's in my left hand.
Two Doctors said that it is unrelated. I found that hard to believe.
Finally, here's a Doctor that mentions the correlation.
Same for me 😢
Thank you that was a great simple explanation of frozen short.
Thank you
thank you sir
very helpful sir 👍
Dr Ebrahim, your videos are always informative and knowledgeable.
What is your experience with RICE BODY formations in the shoulder joint?
Very helpful thank you
That was very accurate and I think the right shoulder is succumbing to it as well. Debilitating.
I had this thing for 8 years, then it changed into a different chronic pain that I belive is for life. You never really recover totally from Adhesive Capsulitis. It's really damaging.
Thanks
Best thanks sir
Thanks sir. Let me ask you a question please. What dou you think about the hydrodilatation treatment with agresive isotonic fluid injection in shoulder joint? Have you ever tried it to your patient and how was the result? Regards from Turkey.
The treatment is to cracking the bone setting
Thank u sir

Can a patient with lymphocytic thyroiditis get frozen shoulder?
Doctor sir am suffering from scapular winging is there any solution sir......
I injured my right shoulder in Sept 2021 and currently have this problem. MRI with Ortho shows no possible tears on the labrum or rotator cuff. It’s painful reaching for things, it feels like my biceps with tear, it’s extremely painful lifting my arm to the side. I have had a cortisone shot with no improvement pain and physical therapy for the past 8 months hasn’t improve mobility or pain. I have also developed a spasm of the shoulder which I call my Tourette shoulder. I am frustrated beyond believe!
Hi hope you did recover
I had rotator cuff surgery and now I have frozen shoulder im getting a mua hope that helps can sleep.
Doc... me da una tristeza no entender sus vídeos 😍
Dear, you can learn English by watching children's cartoons and old TV shows.
Can frozen shoulder help heal with
electroacupuncture? (I'm asking)
Yes with acupuncture and distal acupuncture, with moxa.
Yeah, the pain is severe. Tell that to my doctor who is trying to wean me off the medication that helps me. I had this procedure done and was operated on and I’m no better for it.
I have been telling my Dr after i got adjusted @ chiropractor and he tore my superspanatis.
Nobody will do surgery because of this but i can feel its dislocated
I have this now....all movement hurts..
Proffesser Dumbledore is that you?
👍👍👍

Yeah, I’d give anything for a good nights sleep and to be able to wipe my ass like I used to! Just imagine wiping with the opposite hand that you’ve been used to your whole life! This injury has thrown my life into complete turmoil. I have lost so much including family and now a bit of my sanity. I can’t take the pain anymore. I have fought so hard just to get a doctor to listen to me it’s never about the pain. It’s only about the medication that I’m on. apparently there’s a time limit cap on my medication, but not my pain. imagine living in the world where you’re lumped into a category created by drug addicts only to be denied help from every physician you pour your heart out to!

I've had two frozen shoulders. The 1st lasted 24 months, the 2nd lasted 18 months. Incredibly painful and from my experience there is no treatment that helps at all... except time, a long time. You will find many practitioners who claim they can help your frozen shoulder. If you believe in them, then maybe the placebo effect will help you. That's your only hope. If you want to save some money and still get the placebo effect, try some of those laughable homeopathic drops. But you need to be a believer or it doesn't work. If you're a cynic like I am, or if you believe in science instead of witchcraft, then prepare yourself for 18-24 months of agony.
Me too!
@@teresasugden466 You too what? Two frozen shoulders? Tell us about your experience. "Me too" isn't good enough!
@@opentrunk me too, as in both shoulders, about 3 yrs apart, for no apparent reason. I am active and garden alot, so I do use my muscles. It took me a year for both shoulders, first my right then my left. The left much in worse as in pain and in healing. Had to do cortizone shot for the left.. now I fractured my radial head on my right elbow just 3 months ago from a fall on concrete. Had to have surgery. The frozen shoulder was much more painful! I'm working hard on the physical therapy for the range of motion but it is slow going...I'm 60 now..I hope than in a year I'll be healed again.
@@teresasugden466 Interesting, thanks. No apparent reason is right. It's a total mystery to you, me, and all the experts. There has to be some specific cause just waiting to be discovered. Nobel Prize to the person who figures it out! (Good luck with you elbow)
@@opentrunk thanks!
