    Wait a sec, you’re using a gen 7 tourniquet incorrectly. It’s not supposed to be fed through twice on the friction adapter. You’re applying the gen 6 friction adapter to the 7 and that makes zero sense.

    I actually found some of those studies he was talking about in the episode. C.A.T. 7 tourniquets are still effective on upper arms and thighs, but can lose efficacy after 1-2 minutes as the muscle relaxes and creates more space under the tourniquet. It was really interesting about the amount of pressure needed on larger limbs and its relation to surface area of the tourniquet. If someone has a really thick thigh for example, getting a C.A.T. tourniquet tight enough for arterial occlusion may cause nerve and soft tissue damage, but they can still be effective at stopping the bleed.
    I only read through these studies for about an hour or so and from what I got is that C.A.T. tourniquets are still effective, you just may need to use more than one on larger limbs, and you still need to reassess your patient after a few minutes.
    Over all great stuff! I was really encouraged to go out and look up some of this information and it really did change my opinion on tourniquet types.

      That's why you reassess your patient at least every 5min to ensure you still have HemCon.

    I would have loved to see Scott address alternative TQ designs like the TX3 from RevMedX, which features a ratchening system that allows for very accurate adjustment and a much wider band. Overall, great talk, amazing insights, and really cool hosts.

    As trauma educator for a major health system (and I run ATLS and ATOM), this episode was up my ally. I am in this world 40 hours a week. Great episode. Section at 32:00 about being humbled, I work with Patrick Georgoff (Behind the Knife podcast). I have been in the trauma biz in some capacity for 35 years, and I feel like an infant beside him.

    Pleasantly surprised that initial focus was on presentation of trauma patient(s) to higher levels of medical care than on trashing the use of tourniquets due to incorrect application and removal.
    Exposing all wounds and applying pressure dressings was the preferred go to method for controlling bleeding was embedded in my initial training experience back in 1974.
    I did enjoy learning about the effectiveness of current types of tourniquets available and in use.
    The applying pressure is a topic having similarities to proper application and removal of Military Anti-Shock Trousers (MAST) back during the 1970s and 1980s. Is MAST still being used?

    Do you guys think what Trent asked regarding electrical tape and tourniquets, I was wondering if he might have had a good idea improvising tourniquets utilities electrical tape if you're in the field and don't have cuffs. I think it's possible and could be effective if done well! 🤔

    The newer generation of CAT tourniquets have the single loop design. Not two. It’s hard to tell from the video, but I don’t think that the version of the tourniquet he has was intended to go through two loops as mentioned. Not an issue because the point he is making is still relevant and accurate IMO
