Anytime I upload I get this error responce
"The survey sheet is either empty or missing important column headers.
Your file should have the .xls or .xlsx file extension because Ona uses XLS Forms to create forms.
Not sure what to do? Read more aboutUsing XLSForms"

I have received your data sheet
First remove the "choice filter" column from the survey tab because there is no formula in the choice filter to filter any choice a user chooses based on a question.
2. Now after removing the "choice filter" column and it doesn't work, now change the variable names you created in the "type" column in the survey tab by removing the underscores and Making it short and relevant
