To me, the NIN one gets some extra disturbing points because the man featured in the video, Bob Flanagan, is a performance artist known for gore performances (I recall it was a way for him to regain control on his body, as he was suffering from a cystic fibrosis). Therefore, I am really sorry to say, aside from when he gets killed in the end, everything that happens in this video is 100% real. The drilling of the hands, the smashing of his genitals, he went through all of it, as it was part of his live act. This man's peepee has been through a lot, trust me.

Funny story: I was the music editor at Bloody-Disgusting (eventually Managing Editor) and I exclusively premiered many of these videos, specifically the Cephalic Carnage, Job For a Cowboy, Cattle Decapitation, and Whitechapel ones. It was a blast doing these!

There is a documentary about him called Sick: The Life and Death of Bob Flanagan, Supermasochist which is worth checking out, but not for the faint of heart since not only does it feature some of his performances (I think it was cut a bit in the UK for some of the really extreme moments), but it also shows his death in hospital.
I find Korn's Insane video disturbing because taking photos with dead family members was common practice. Bodies would start decomposing in peoples houses, plus John Davis was a mortician and he said dealing with dead kids bodies fucked him up.

Honestly Obscure is one of their tamer videos to me. Mazohyst of Decadence and The Blossoming Beelzebub are way worse and they're not even half as gory.

Ahh dir en greys obscure, the video of theirs I found as a young teen which started my journey into what is essentially my favourite band 15 years later. Their albums uroboros and dum spiro spero are essentially avant garde progressive death core. Very good stuff
The reason for the Nine Inch Nails video being 'well acted' is that it wasn't. As some people already said in the comments, the tabledude was a hardcore performance artist and everything in the video (except only for the death scene) was actually real and not him just playing pretend for the camera. He also appeared in a few other music videos, one of them having him hammering a nail through his peepee's most sensitive part and bleeding right on the camera itself, which was also probably real.
My number one pick is “Encased in Concrete” by Cannibal Corpse. Visually, it’s pretty much a standard metal MV but those close up shots of the guy frantically moving around as concrete is poured onto him is fucking terrifying, and the lyrical content makes it so much scarier. Encased in Concrete was the first Brutal Death Metal song to actually make me scared
My buddy Klint was in the Dying Fetus (he’s in a few of their videos) and FGR music video (guy doing inflicting the pain in both). It’s funny because he’s actually a really nice guy. Getting the background of how they did the effects was pretty interesting!

Aside from what lots of people pointed out from the NIN video (Regarding the late Supermasochist Bob Flanagan) is that the video was also part of a short movie using most of the songs of the Broken EP. `Happiness in Slavery` is in the middle it was already messed up but the last song, `Gave up` in the movie goes even harder to close out the story (Don't even wanna say much to avoid spoiling) , not sure how you'd go about to find this, shouldn't be too hard now, had to use torrents a few years ago to download it.

Hellyeah! The last 4 songs and videos are absolute gnarly bangers. Notes- Cephalic Carnage is a "Hydrogrind" band from Colorado in the US, the whole "Misled by Certainty" album that the song "Ohrwurm" is on is phenomenal. Been a while but they're apparently working on a new album finally, super underrated band. The JFAC "Tarnished Gluttony" video is based in Lovecraftian Horror Mythos, an offering to the Great Old Ones (Main adult character has Gills shown at the end). Cattle Decap definitely takes the cake with "Forced Gender Reassignment" as one of the most extreme. Honorable Music Video mentions: Precaria- they had a music video with a bunch of real footage from Tibetan Sky Burials and other Latin-America footage of live body Crucifixions during certain holidays and ceremonies, no longer available (was on youtube for a long time). Awesome Black Metal outfit. Lichtblick- Ten Cent Pistol, about the Austrian and Global Heroin Epidemic. Excellent band. Lifelover- Androider, used to be a video that was a bunch of real suicide and self-harm footage (may have been fan-made but was really well done). Amestigon- 358, much more esoteric with occult/ satanic overtones, some of the old footage therein can be seen as disturbing by some. Also an excellent band. KFR- Multiple music videos where real human bones are used/ present, and Maxime Taccardi is known as the "Blood Artist" making amazing artwork with his own blood, that often finds in scenes in his music videos.
Another one is Anthrax's "Blood Eagle Wings," the song itself is one of my favorites but I wish I had never watched the video, it's literally an 18+ video.
COF's From The Cradle to Enslave is always mentioned as their most disturbing video, and sure it's the most graphic, but I've always thought the video for "Babalon AD" was much more disturbing (it's based on the movie Salo, which explains it).
Yes! I remember watching FTCTE and learning about the B movie filmed by Danny about that time. Searched for it and was a f... chaotic movie. Super gore and pixelated. Amazing. But as you said, the feeling I got out from "Babalon AD" was more vague and archetypical. Well mentioned!

Dir en grey videos are awesome but I never felt like Obscure was actually 'disturbing' (American's view on nudity is disturbing to me, though ^^'). The one Dir en grey pv I might consider disturbing would be Vinushka because there is nothing more disturbing than reality...Mazohyst of Decadence might be a bit disturbing as well for similar reasons (no real images in that one, though) - both are very different from this lists understanding of the word, it's just my personal view - I would recommend listening to the songs with the pvs though if someone who only knows Dir en grey from Obscure is interested :) [Mazohyst of Decadence is from 1999, Vinushka from 2008]. Going with the kind of gore displayed in most of the videos here I would have to say Dir en grey's live shows are far more disturbing than any of their videos XD Tarnished Gluttony is actually the only video on this list I found disturbing when I first watched it. [Forced Gender Reassignment is just some nice, old-fashoned social-revenge fun ^^ Also good lyrics]. One of my favorite music videos for Halloween is PORTAL Ω CURTAIN. That Obscure PV was heavily censored because it was meant for TV screening, I think. The original has baby-head eating and more vomiting clips. So the version you have watched amd reacted to, was PG-13 compared to the original haha
Forced Gender Reassignment definitely takes the crown on this one, but since I love the absolute shit out of Cattle Decapitation (monolith of inhumanity being one of my favorite albums of theirs, where this song is from) it's also one of my favorite videos.
I'd almost argue that the bad FX in Obscure are part of what makes it disturbing. It looks bad in such a way where, everything looks so off and out of place, especially mixed with the real people.

I wonder if you checked out the "normal" Obscure or the extended one, the extended one has a genuinely creepy moment in the added keyboard drone part in the middle (especially once you realise that the song is seemingly about abortion). It had quite a shock value back in 2003 but you can find much worse now, including from dir en grey. Killer song though, even if they never managed to play it live quite right. Also, yay, NIN is here as well, that's another really important band of my adolescence. The whole Broken movie was really disturbing.
The exploding heads in Archspire's video for "Drone Corpse Aviator" are pretty fantastic. Try not to let the drummer melt your face, otherwise you might not see the exploding heads.

Saw a music video years ago (I can't remember the name of the band or the song) that opened up with film footage of headless human corpses hung upside down like cattle, and during the video, you see a projection of a man being decapitated, and it looked *VERY* realistic...

Dir en grey videos are awesome but I never felt like Obscure was actually 'disturbing' (American's view on nudity is disturbing to me, though ^^'). The one Dir en grey pv I might consider disturbing would be Vinushka because there is nothing more disturbing than reality...Mazohyst of Decadence might be a bit disturbing as well for similar reasons (no real images in that one, though) - both are very different from this lists understanding of the word, it's just my personal view - I would recommend listening to the songs with the pvs though if someone who only knows Dir en grey from Obscure is interested :) [Mazohyst of Decadence is from 1999, Vinushka from 2008]. Going with the kind of gore displayed in most of the videos here I would have to say Dir en grey's live shows are far more disturbing than any of their videos XD
Tarnished Gluttony is actually the only video on this list I found disturbing when I first watched it. [Forced Gender Reassignment is just some nice, old-fashoned social-revenge fun ^^ Also good lyrics].
One of my favorite music videos for Halloween is PORTAL Ω CURTAIN. Definitely disturbing but more on the artistic side [Dr. Caligari meats Poe].

That Obscure PV was heavily censored because it was meant for TV screening, I think. The original has baby-head eating and more vomiting clips. So the version you have watched amd reacted to, was PG-13 compared to the original haha
Forced Gender Reassignment definitely takes the crown on this one, but since I love the absolute shit out of Cattle Decapitation (monolith of inhumanity being one of my favorite albums of theirs, where this song is from) it's also one of my favorite videos.
Death Atlas has been on repeat in my house since 2019 haha. Cattle Decap is so fucking good.

I'd almost argue that the bad FX in Obscure are part of what makes it disturbing. It looks bad in such a way where, everything looks so off and out of place, especially mixed with the real people.

SHINING - Vilja & Dröm is the most disturbing video I've ever seen, but in a real sense, because the footage is all real. Maybe not a good idea for a Halloween video because it is deeply depressing.

I wonder if you checked out the "normal" Obscure or the extended one, the extended one has a genuinely creepy moment in the added keyboard drone part in the middle (especially once you realise that the song is seemingly about abortion). It had quite a shock value back in 2003 but you can find much worse now, including from dir en grey. Killer song though, even if they never managed to play it live quite right.
Also, yay, NIN is here as well, that's another really important band of my adolescence. The whole Broken movie was really disturbing.
I think Obscure almost always sounds good live.

The exploding heads in Archspire's video for "Drone Corpse Aviator" are pretty fantastic.

Maybe it's because i see metal as an antagonist to ignorance, whilst ignorance leads to violence.
I may have a therapeutic view on the metal genre for me personally, not dissing no one, but on the end of the day i know if it pulled me down or lifted me up.

Saw a music video years ago (I can't remember the name of the band or the song) that opened up with film footage of headless human corpses hung upside down like cattle, and during the video, you see a projection of a man being decapitated, and it looked *VERY* realistic...

The whole Broken video is so disturbing. It was also filmed by Peter Christopherson, aka Sleazy, of Throbbing Gristle and Coil. He also designed Pink Floyd's 'Wish You Were Here' and 'Animals' as part of Hipgnosis.
