What if one has the neurosyphilis symptoms but the test for syphilis via spinal fluid shows no antibodies? My son is in the hospital for over week, second time this year. His wbc is super high, protein in spinal fluid is sky high, he has head scalp rolls. Eye issues, pain in neck and back, bugging eyes, weird pupils, hydrocephalus. He is presenting as meningitis but tests come back negative. I believe he has neurosyphilis. Is it harmful to treat if the patient really doesn’t have neurosyphilis? Everything seems to point to it. Please advise. If he is having a false negative test, how else can they test? What other disease mimics syphilis besides meningitis? Please help!

The Centers for Disease Control and Prevention and European guidelines on sexually transmitted diseases (STD), consider various combinations of reactive serological test results, including a reactive CSF VDRL test, together with an augmented CSF leukocyte count and evidence of intrathecal immunoglobulin production with or without clinical manifestations to be indicative of neurosyphilis and I feel your child clinical scenario(both VDRL and RPR ) does not meet the above criteria so your child case is strictly negative to syphilis...The doctor has to make a differential diagnosis to rule out the clinical picture.

The VDRL test isn’t always accurate. For example, you may have false-negative results if you’ve had syphilis for less than three months, as it could take this long for your body to make antibodies. The test is also unreliable in late-stage syphilis.
On the other hand, the following can cause false-positive results:
HIV
Lyme disease
malaria
pneumonia (certain types only)
systemic lupus erythematosus
IV drug use
tuberculosis
In some cases, your body may not produce antibodies even if you have been infected with syphilis. This means the VDRL test will be inaccurate.
The antibodies produced as a result of a syphilis infection can stay in your body even after your syphilis has been treated. This means you might always have positive results on this test.
