How do you explain the sudden cut at 0:57 where you go into the door without a mask, and after the transition, you suddenly have a mask on? If you were to press e, the sound of the mask comming off as well as the animation should have been played

Since the monsters are a representation of Simon's trauma and depression, it makes sense that the Doctor, as his therapist, also has to go through some of them

This speedrun may be fake, or he changed the game settings, or he is just using bugs. 1) You cant crawl at that speed. 2) You cant go that fast unless you're running. 3) You have too much range to use doors. 4) You used doors beetween walls, 3:40. 5) 2:30 This part of the game is extremely dark, you have changed the game. 6) You must be a lot closer to climb a stair. 7) The city has fog. I dont call this fake by itself but there is some incorrect stuff in the game, you should try to not make it too easy for you.
Seemingly high speed and high range on doors/stairs is because of high FOV, it's not prohibited to modify it (I think). Using door glitch is also allowed, you can see others using it in their runs, please look it up on the leaderboard. Using brightness boost is also allowed. He's not "making it too easy" for himself for no reason, all of this stuff is LITERALLY allowed by the rules (or at least not prohibited).

Source games have sectioned rooms set to not render unless you're viewing them. This player had their FoV so high it probably messed with the system.
So in this case the flickering is just the after image of the monster that was on his right
