That desire ends up hurting him, and he becomes lost in the darkness and the smoke of the city with his last reach for the stars.
The man is moving his hand (off camera, you see the arm jerking around), at 0:33, her face is a symbol of her presence, it means that despite the act is sexual, physically sexual, its a pure thing, he cares. But it still is desire. Btw EVERY film David Lynch has done, EVERY SINGLE ONE is sexual or has a moment about this relation with desire. (except for the Straight Story and Dune, that approach things differently...its still there, but much more distant) - Its his main subject.
