Your plant might have root rot. I think you should check the roots, and if they're rotting, just reroot it in perlite. That often does the trick! Also, when you repot your plant, use a well-draining soil mix (more perlite + coco chip), and wait until the top 5 cm of soil is dry before watering. Hope that helps! ^^
