The D4 light flashing is the transmission error light. The check engine light is well the check engine light. I had this exact same problem. It was the VSS (Vehicle Speed Sensor) fixed it right away. 2 bolts and a $13 part.
At this point, you must have fixed your problem, But I agree with ToxicBeast71...SPEED SENSOR most often is the culprit. Usually you will see light and the vehicle will not shift into the final overdrive gear. Replacing the sensor returns the overdrive function and saves you gas.
Anyone watching this in the future. If D4 is flashing, all that’s saying is the TCM (not the ECU) has stored a code. It doesnt mean you have transmission problems, just that the computer is detecting an issue relating to the TCM. Make sure your TCM is working first before you do anything else. A TCM will tell a car to shift like hell or not shift or not start simply because its malfunctioning or a wire connected to it is. There could be nothing wrong with the your actual transmission.

Solution pls :- My D4 light always start to blink when the gear change to 3 or 4, but if i off the engine and start again it will stop but when it change to 3or 4 gear is start to they blink again but my speedometer is not work, i need your advice
Transmission shift control solenoid valve on the Transmission. Can take it out and clean the gasket from gunk. That light only blinks due to an issue with the Transmission. It'll still drive but accelerating would feel sluggish. I need to do that with my car at the moment.
I've had this exact same problem. After resoldering the major components of my ECU, the problem vanished and I even felt the car more powerful.

That happened when I replaced the solenoid in my transmission it fixed the problem but it triggered a cold it happens if you don't use a scanner

My Accord 1996 Ex-R blinks the "d4" light and it stop pulling after a few minutes!
I change the oil but the fault continues.
