Dispelling the Fear: Erythritol and Cardiovascular Risk

  • A discussion of the scientific article published yesterday.
    The research title:
The artificial sweetener erythritol and cardiovascular event risk
Summary: this was a poorly designed study with the authors extrapolating beyond the data.
This research study investigates the long-term effects of erythritol, a commonly used artificial sweetener, on cardiovascular disease risk. It explains that although artificial sweeteners are generally considered safe, little is known about their potential long-term health effects. The study found that erythritol, when consumed at physiological levels, enhances platelet reactivity and thrombosis formation. The video provides an overview of the study's methods and findings, including the association between circulating levels of erythritol and the risk for major adverse cardiovascular events. It also discusses the potential implications of the study's findings and the need for further research to assess the long-term safety of erythritol.

This is exactly why studies need to be peer reviewed. I have listened to a handful of RUclips experts with a variety of opinions on the subject. This is the first review that is actually showing and explaining the findings of the study. It is so frustrating when you want to do the right thing and consume safe foods. Perhaps a proper study will be conducted in the near future.

Don't fool yourself, peer-review isn't that all what it used to be. It's absolutely been hijacked just as everything else has been. It is not the golden stamp of approval that it once was. Practically anything that It's on The Fringe is automatically thrown in the garbage. And people who just have a name behind them are often put through. There's been groups of people that have written up bogas studies just to show that peer review is bullshit. Studies such as claiming that dog parks condone and perpetuate rape culture.... Along with countless other ridiculous things. They all got peer-reviewed and put through... Because they all pandered to some type of Hot topic talking point

Your comment might be misleading. I doubt the article mentioned wasn't peer reviewed, afaik that's the standard for any scientific institution that publishes in major scientific magazines.
But peer review isn't exactly bullet proof, rather far from it.
Safety rules never guarantee safety, it's that simple.

Another issue with epidemiological studies like this is that they do not show the direction of causality. Does high consumption of non-caloric sweeteners cause problems like diabetes or obesity...or do people who have diabetes or obesity choose non-caloric sweeteners in order to deal with their diabetes or to lose weight?
The gold standard is human randomized controlled trials, in which what the subjects eat is determined. But even there, problems can damage the study results. For instance, a study might decree a diet that matched calories between test and control groups, by adding in some extra calories elsewhere for the test group using a non-caloric sweetener. Sounds reasonable, right? But that would not tell us what happens in the real-world situation, where people are choosing non-caloric sweeteners in order to reduce their calorie intake. And the fact that extra calories were added with increased amounts of some other food is itself a confounding factor.
There are methods for determining causality even when it is not possible to construct a randomized controlled trial -- these come from the field of econometrics, which deals with human subjects in situations where it would be unethical to randomize, or where blinding wasn't possible (e.g. in a medical treatment study where a placebo was not feasible, such as when the test group was receiving cytotoxic chemotherapy -- patients will know if they are receiving chemo or not, and it is not ethical to give fake chemo that causes deleterious side effects, but has no anti-cancer action), or where people assigned as controls might decide to take actions on their own that mimic the test treatment. Designing such studies is tricky but doable. Esther Duflo, Abhijit Banerjee, and Michael Kremer were awarded the Nobel Memorial Prize in Economics in 2019 for their work applying econometrics to study interventions to alleviate poverty.

Thank you Doctor. Excellent review. The first question that came to mind when hearing about this study was how did they account for the fact that this population was already at high risk for cardiac events. The answer seems to be that they didn't. That they didn't even confirm that the population was or was not ingesting high amounts of erythritol seems incredible. What we seem to have is another associative study that doesn't prove causation. Certainly, another example why you must never place a large amount of faith in any one study. Remember the big study that had all the cardiologists prescribing omega 3 fish oil to stem cardiac events. Analysis of follow on studies could not confirm the effectiveness of the omega 3 in reduction of the cardiac events.

While this definitely needs more study, the industry's reaction to this being nothing seems very flippant.

Study is flawed because of selection bias. What group of people are most likely to be consuming "sugar-free" sweets? Overweight people who by definition will be at a higher risk of adverse cardiac events. So no big surprise to find a correlation between people ingesting erythritol and cardiac events. That said, I am a bit concerned that unlike other artificial sweeteners that pass through your GI tract, 90% or erythritol gets absorbed into your blood stream and ends up in your urine via the kidneys.

I was for about a year, adding three (3) packs per day of the yellow Splenda to my daily Greek yogurt concoction when I learned about the possible (negative) effects it may have on cardiovascular issues. Since my family has some history of high cholesterol issues, I decided to look into something else, and switched to Splenda's Monk Fruit product. Literally one day after the switch, I learned that it had Erythritol in it, and when I researched that and immediately found the press reports on this study, I thought, "Oh damn! I went from maybe killing myself with artificial sweeteners, to absolutely killing myself with them!" 😱
I read several alternate opinions of the study, and many/most of them offered the same questions you do in this video-- basically questioning the voracity of the study. So this is a looong winded way of saying, "Thank you." Maybe you can do a video offering your opinion about the class of sweetners that fall into the category of sugar alcohols-- that seems to be another area of general controversy/disagreement.

I am a keto dieter and only have coffee in the morning , may be a factor . All in all None of the 8 or so doctors of medical facilities could find this . I am very happy to find this is my problem and will use Stevia without Erythritol. Hope this helps .

I don't like the condescending attitude in this review. I don't think any "Doc" should encourage or imply that there followers should not pay attention or dismiss this study. I feel like a lot of people on RUclips want to act like they're smarter than the scientists performing the study. The study is not flawed it's simply ongoing. The fact is the study is not complete and more important relevant trials are taking place now. First there was a "starting point" which showed very strong corellation. Then a "pilot study" w/ingesting erythritol. This is how good studies progress. They never said they proved causation. What they did say is that normally they don't warn people (sound the alarm) this early on but the results were so alarming, that they thought it would be in peoples best interest to give a warning of potential dangers now. I know I'm glad they did, because I will throw this garbage mass produced synthetic sweetener in the trash (regardless of the final outcome.) I never realized what I was putting into my body. Erythritol is made mostly from gmo corn and 95% is manufactured in China. It's so cheap they use it as a "bulking agent" and sneak it into junk foods and other better quality natural sugar substitutes. I have seen many opinions on this study and the responsible people are at least warning people with pre-existing conditions to either avoid or cut back on consumption for now. Why make a statement that this was a poorly constructed, strange study ..this is only your opinion and encourages people to ignore any warnings. While the study is still ongoing and the verdict is not determind yet? What if some of your followers have pre-existing conditions like obesity, heart disease, high blood pressure or diabetes. Also if your body produces it as a response to health problems , then why would we want to eat a synthetic form of it? (Look at how many unhealthy products remain on the market today...it takes forever for the truth to come out... no one needs to eat it, why put yourself at risk..just use something else )
---Bottom line erythriltol may turn out to pose a health danger, studies are leaning this way and no one knows what the ongoing trials will show definitively...

Hmm I really wanted to like erythritol as aspartame, sucrose/sucralose, and saccharin all damage the gut biome among other problems. And I know Stevia is insanely sweet so they dumb it down with erythritol. What higher quality sweeteners can you recommend?

A complete garbage "study", that had a preconceived outcome, and was designed to give the desired results.
