    I Apicoectomy since I have a large mushroom post in my upper last molar.
    The Edodontist I saw messed me up. He saw I had a big post like a mushroom upside down, but still decided to remove my crown & try to drill through the very hard alloy which e should have dental-cal but don't wi to B messed u more. I will maybe have to pay for some of the work out of pocket, but I would then k Apioceental insurance.
    What has me very upset is the Endodontist didn't suggest this procedure when he KNEW he couldn't drill through the alloy metal of the pst to get to the root infection.

    I hear root canals rarely work & can cause the damage to other body parts.

    Thank you for your very informative presentation Dr. Allen Ali Nasseh, I would like to ask for your opinion about Ketac-silver (Silver Reinforced Glass Ionomer) as a retrograde filling material. Thanks in advance!

      +Fotis Roilos Ketac Silver and other glass ionomers as well as bonded composites work great in vitro. Unfortunately, these products are moisture sensitive and have shown poor seal in vivo. The periapical environment is very moist and a hydrophilic material is needed. BioCeramics are the best for this purpose. MTA was the first but this combination I describe is a way to make the most of a newer formulation (BC Sealer/BC Putty) that is much easier to handle than MTA. Good luck!

    Allen, what if one encounters a non-vital tooth with apical rarifaction with calcified canals that cannot be negotiated? Can an apicoectomy with retrofill be done to save the tooth?

      Of course. However, apico has a higher prognosis if endo is done first. However, if canals can't be found coronally apico might be the only option. Such decision is normally done by the endodontist on a case by case basis.

    82-year-old patient has stopped 70mg oral bisphosphates 26-days ago after taking for 84-weeks, and reveals apical infection # 4 around previous RCT; Buccal draining near sinus tract. Tooth mobility is acceptable and we prefer NOT to remove # 4. What is risk of ONJ following apicoectomy with retrograde filling? Is longer washout of bisphosphates necessary? Thank you Allen for excellent video.

      Highly recommend that you speak to the patient’s physician. This kind of advice should not be given over RUclips. Chance is relatively low compared to extraction but is still there.

    How many times can you get a apico on one tooth? And is a apico just buying time before the tooth eventually has to be extracted?

      Everything in dentistry is buying time until the tooth is extracted in the same way everything in medicine is temporary until we die! That's healthcare for you in a nutshell! So, the goal of both dentistry and medicine is to postpone the inevitable for as long as possible. A visit to the doctor doesn't make us immortal and a visit to the dentist doesn't save any tooth forever either. An apicoectomy can be redone only if the original source of failure was not addressed properly through the first apicoectomy. A CBCT and an experienced surgeon is required to make the right call regarding the best approach. Keep in mind the alternative, which is implants, is not forever either. Cheers!

    CAN WE KEEP GP SHORT IN THIS CASES AND FILL IT WITH BC SEALER IN APICAL 3MM OR 6MM?

      If you plan on doing a combined orthograde and retrograde technique together you can. But you can't do it the same visit as the sealer will need at least a day to set.

    what iodine do you use? can you disinfect with bleach?

      This is a non-commercial optimized iodine formula. It's not available other than a compound pharmacy. You should not use bleach as it's highly caustic in a surgical site. You can use 2% Chlorohexidine, however, and that should be fine. Simply rinse it out with sterile saline before placing the bioceramic.
