    Thank you so much. I had a hip arthroscopy and labral repair 2 years ago, and never healed. I gave up after awhile, but it hit me earlier today... im only turning 17 this month... i cant do anything on my bucket list, its time to get this figured out.
    1.5 years of PT. I really hope this helps, but i will say, this is the most information ive found over it yet. Appreciate you Man!

      Look into a good physiatrist esp at a hosp w/sports medicine. Your age will work for you but get good advice. I think the PT who does these videos offers virtual consults too.

    I'm 34 and been a high-level athlete my whole life. I tore my hip labrum in Jan 2023 and it's now over a year later and I'm still miserable, not able to go more than 40% in my workouts or play sports. I cannot wait to get surgery and end this pain and debilitating lifestyle once and for all! Surgery is definitely the right route, for some.

You know your body better than anyone, including doctors. We specialize in helping people avoid it if possible as surgery can be very risky for various reasons. However, sometimes there is no avoiding it.

    How Dr I'm 64 I have a hip and femur replacement. I fell two weeks ago at the bowling club . I tripped over a edge. I'm not on a medical aid. I have pain in my hip joint since the fall. I play bowls regularly and would love to continue to play. I'm watching your videos to gain more knowledge about this problem. Thank you so much. I'm walking with a crutch at the moment.

    Hi Dr Middaugh, I had hip athroscopy surgery in 2015 for a labrum tear, and I am still experiencing groin pain until I watched your video on muscle imbalance and squeezing the glutes while walking has made a huge difference.

    This is the missing info I needed about my hip labrum tear-I knew in my heart a body could heal, and I was getting it mostly right, to where I never felt pain for 5 years, even as active as I am. Recently I flared up a bit...I think my gait ( walk) must not be quite right. Recently did more hiking in mountains than usual for me. I feel so much more optimism again-thank you for this vital information.

    Thank you!!!!!!! I had fai repair in 2013. Re-torn last year but mri also showed arthritis. Ive gained 50 lbs because it hurts to workout. Specialist at the U of U says he wont do surgery and completely ignored the mri tear finding or my symptoms. Your video is the closest thing to hope Ive had. Im only 42 so i cant imagine living with pain like this for the rest of my life.

    Thanks for the info! I have two tears in the hip labrum, but my pain isn’t when I exercise, but when I’m at rest, either siting or laying down.😢 it also hurts really bad if my leg goes out or in to the sides. I can bike or walk without pain. Any comments on that? Thank you so much!!

    Your a breath of fresh air ! in y 70s live alone with 4 dogs to walk ! , left knee minniscus tear right knee patella popped out ! rig!ht hip just as you say , after listenig to you I will do as you say I am going to get stronger and get back to a better mobility , thank you !

      I am a very active 73-year-old. I walked 3 1/2 miles in the morning at a pretty fast pace. A couple of weeks ago 3 miles into my walk with just a half a mile to go, my right leg out of nowhere started hurting with each step and getting worse each step. By the time I made it home I could barely put weight on it. I grabbed the crutches out of the basement and use those for a little bit and now I am using a cane. The MRI showed a torn posterior labral tear. I have PT set up to start July 9. I would really like to avoid surgery. Any suggestions beside your video information would be appreciated.

    Great and informative video, thank you. Yes I have a labrum tear, confirmed with arthritis too.. I’m only 54 and a Sprinter… does this mean this is the end of my high impact sport, or crack on as they say surgery will come at me regardless… I’ve also just had a steroid injection and feel amazing!

    Thank you for your information. I fell while recovering from back surgery and ended up have labrum hip surgery on both hips. The right hip labrum was replaced with a donor's labrum when the first surgery did not hold up. I have been having groin pain and I cannot lift my right leg. Recent MRI indicates another tear. Doctor recommended a hip replacement. I excercise 4-5 days a week and I am wondering if I walking fast and running on treadmill is making it worse.

    My doctor said I have a (moderate) cam impingement, and that it will make it very unlikely/hard to heal the labrum on its own without surgery (because the impingement will just keep smashing the labrum and prevent it to heal)
    What are you thoughts on that ? Do you think it is still possible to heal "naturally" a labrum if there is a FAI ?

    Hey Dr. Middaugh. I am 38 years old and I have been having hip/groin pain for 15-20 getting progressively worse. I am very active, exercise regularly and enjoy hunting and hiking a lot. About 10 years ago my groin pain got bad enough that I went in and the doctor told me I had a small hernia however after surgeon consult I was told I didn’t and that I probably just had a pulled muscle. A few years later I got really into triathlons and had to go back in for groin pain. Doctors told me same things. Then in the Spring of 2021 I was diagnosed with FAI syndrome with cam and pincer impingements and a torn labrum. I proceeded to have the bone deformities and labrum fixed however the surgery did not go well. The surgeon was pretty inexperienced, and he took over 9 hours. I ended up with nerve damage with my non surgical leg that developed into CRPS. Recover was a pretty long road just to get me through the nerve/CRPS let alone the hip recover. I did 9 months of PT and most of nerve/CRPS has dissipated however every time I start to push my exercising and or hiking my groin pain comes back. Its currently flared up as much as before my surgery with pretty intense pain in my groin. After watching this video everything seems to make sense. I just wish I would have seen this 20 years ago. A week ago I thought I was destine for a hip replacement which was so depressing, but now I am hopeful that I might be able to avoid it. I just hope I haven’t done too much damage to my new labrum and that I can figure out how to correctly use my hip joint. Definitely wish I lived in TX for direct treatment.

      I am so sorry your 9 month surgery process .
      The pain you are experiencing sounds like it keeps flaring up and coming back. That is consistent with muscle imbalance in the hip where your quadriceps and hip are over worked and your gluteal muscles are weak. Here are some helpful videos:

    Hey Dr. Middaugh, thank you for that informative and helpful video first of all. I have one question. I have a labrum tear in my hip and am about to decide if I will have surgery or not. You mentioned that there might still be the tear seen on an MRI but people feel fine because of strengthening the area around. I always wonder if this might mean, once I am old and I might not be able to keep my hip as strong as in younger age, would I then have to deal with the problems and need a surgery?     This is for real. I can't afford healthcare right now and I hadn't completely lost mobility either, so doing the small movements & exercises he suggested felt really good and relieving. I am just about 30 so knowing maybe I might not need surgery is an enormous weight off my shoulders. One day after doing the movements and strengthening - no joke - I feel better. I can't go downhills quite yet and I'm only doing the very slow and shallow squats but the focused movement of the glutes is key. I am very much relearning how to walk and I will keep this experience with me the rest of my walking days. Pretty wild.

      Hi Angela, We are so sorry that you are suffering from a hip labrum tear! We know how painful they can be and hope you can feel some relief soon. Offloading is the best thing you can do when you have a flare-up. It is best to try to rest the hip as much as possible until the pain subsides.
      Hip labrum tears take a long time to heal and it is very important to be patient throughout the healing process. We know that can be easier said than done but you don't want to exacerbate the issue by trying to do too much.

      Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity. It’s best to do these exercises every day throughout the day. Aim to tire out the muscle without losing form or control. Consistency is key. Always avoid working through pain; soreness and mild discomfort are normal, but pain is not.

    I have been back to doing yoga recently after being diagnosed with a small labral tear in August… I think I’m developing bursitis now. Is this common and will it go away? I have no stability issues or pain when putting weight on that side.

    Thank you for this video. I wish my physical therapists would focus on things like functional movement. The several physical therapists that I have been to just massage my leg or hip and then give me to an inexperienced person to have me do exercises that I could just do at home. :/ I'm going to check out your other videos!     So April 2021 I had an mri on my left hip. The results were
    1. Partial-thickness tear of the distal left gluteus medius tendon.
    2. Focal anterior left acetabular labral tear.
    A few months after a slide down my stairs.
    In July 2019 an X-ray showed Enthesopathy of both greater trochanters. Left much worse that right and I have had increasing pain in the left that has affected the front outer of my left thigh since 2004. It would start after long standing or walking in excess of an hour until in 2019 it was a mere 5 mins and my thigh would go numb and lost all strength to stand. Also in 2020 I was diagnosed with Psoriatic Arthritis with major tendon involvement.
    My question is, are the tears caused solely by the fall or is more likely that the bursitis (likely caused by the Psoriatic) is the cause. And what you describe in this video and in the exercise videos will they most likely still keep me?
    Been to PT off and on since the bursitis was diagnosed. I’m still having major issues.

      Sorry to hear about you tendon and labrum tears TransformedByJesus :-(
      In a case like this, any sort of trauma like a fall is typically the event that pushes and already involved situation over the edge. It's likely that you've had a muscle imbalance developing for a while. You've got to get better glute strength. Specifically for you, avoid using the side of your glutes and focus on the back, most posterior part. You might need to start off slow to allow your tendons to catch up in strength so you don't get tendon problems while addressing the muscle imbalance.
      You probably need to avoid any sort of hip flexor and hip abductor exercises - these are commonly offered at PT, but they're not appropriate for a tendonitis issue like yours.

      Thank you so much. Gives. Me hope. I injured my labrum through line dance classes I taught 2 times a week. That was 2 years ago. Could not sit at all much less dance. Slowly I'm healing and can atheist walk, sit and sleep with knees together. I am starting physical therapy and we are working on many things you suggest. A long time ago I used to do Pilates on the machine. I remember how we did leg circles and alot of core work. Was wondering if these leg circles eould hurt me or help me at this point in my healing. The last thing I want to do is re injure my labrum and be back to square 1. Any thoughts on this???

    I have been in pain for years, after a 6+ month battle with sciatica...pain moved to my hip and groin. I refused a hip relationship replacement and have been exercising for years. I'm better, but still not 100%. I am 71 yrs young. I swim every other day with the aqua jogger. I'm trying to strengthen my glutes without impact. I did your sitting and standing exercises and WOW!! My butt is sore! Can you tell me if my swimming (running, skiing, scissoring, and twisting in deep water) is good for this? Thanks for your great videos!

      Hey Karen, glad you were able to get your hip better without a replacement! Working out your glutes is great! These exercises can definitely be great as well, but it is very important that you feel your glutes being the dominant muscle for the exercises. You should always feel like your glutes are tightening with these exercises. If not, you may be using the wrong muscles and might need to choose an exercise a little easier so that you can concentrate on using your glutes. Hope this helps and keep us updated!

      I also have this in both hips but did debriding then labrum reconstruction . Two years later heteophrioc ossification developed in right hip and I have been receiving hip
      Injections that are beginning
      To be less effective each time for the past 3 years. I continue to hear with Kaiser is that I will need a hip replacement by 55 (8 years away) . Both hips are totally bothering me. The best to you!

    I am 34, hip started hurting somewhat out of no where, my main question is, the pain much more on the side of my hip, not the front, but my Dr doesn’t think it’s bursitis because the outside hip area isn’t tender. My range of motion is basically non existent when I lay on my back and try to lift the hurt hip upwards. Is it possible this is a torn labrum since the main pain is on the side and not front of the hip?

    I'm doing your exercises 2 months almost 12 hours a day - every single minute, doesn't help a lot, only to survive to the next day and is not a permanent pain free solution. May be it will help with minor tiny tear and no hip impingement. But it was good to try.

This is not only completely unrealistic, but hypothetically if you were exercising 12 hours a day Jon stop, this would not only screw up the side you are trying to fix, but also screw up the other side too! There is such a thing as too much. Or haven’t you heard?

    i struggle walking with the hip tear it happened 2012 its qone worse since i stopped weightlifting really locks up struggle walking i saw the vid you did about glute strength and the walking so practicing those i used to deadlift and i think its gone worse cause i stopped nearly 3years ago thanks for the vids they are fantastic gives me hope thankyou

    Talking about symptoms of labrum tear, you said the pain is felt in the groin, in the front. But, It is actually not the case for posterior tear or superior tears.

    It´s exactly the pain I feel, when I get up or when I lean on my wright leg and when I stay more than 20 minutes walking or standing up. I´m seventy and I bought two canes and using them the pain is almost zero. I began to cook sitting on a bench and sweeping the same way. I found your videos on hip arthritis and thought it may be that, but now probably it is the labrum because in spite of my age I can move my legs and hips easily as if I were 40. I remember having my hip stuck with intense pain once in a while since I was very young and I had to wait a few minutes till I´ve got less pain and then I could walk again. I shall follow your advices in the video because I started the exercises for hip arthritis and I felt a relief but when I began walking, after 15 minutes the pain appeared again despite using the canes. I have to calm down and avoid being standing up the most and when the pain goes begin with the exercises.     I have been looking for an answer for years. My thoughts on surgery are that if my hips are out of balance and I have the tear it will only be more surgeries. I have watched people around me get hip replacement the the other then knees then back

    i got one (i assume) playing tennis in 2014. went to all the physio and xrays and nothing really helped. couldnt strengthen the muscles. was never painful, just unstable and weak. kinda of got used to it and now it feels really weak all of a sudden and like its floating again. havent been very active lately and its really discouraging thinking i might be destined for surgery. hoping these youtube guys help me. thank you.

    Hey doctor, I am a 630lb squatter (powerlifter). I went line dancing, and hurt my hip. Went to the doctor, got an X-ray , and was told I have a Cam impingement, which likely lead to a labral tear. FADIR test was positive...
    The doctor said to give it 4 weeks to see if it begins to heal-- and if it doesn't, then surgery is the route we will go. It has been 3 weeks, and it feels significantly better- however my fear is that I will never be able to compete in Powerlifting again. Do you think that if this heals naturally, I will ever be able to squat heavy again?

    Just found out I have a labral hip tear. The surgeon said I should probably quit high impact sports which seems depressing in my early 40s. Is there a way to overcome this and continue to play sports?

    I’m experiencing a lot of depression right now due to this, it’s been hurting for months and I was hoping it would go away. I already do a lot of resting because of fatigue. Was thinking of going to the pool to take some pressure off so I can get some movement

    Ironically, my doctors recommended that I do Not have the surgery. One: they said there is no guarantee it will get rid of the pain and two: the surgery will need to be performed again in 15 years as it is not a permanent solution.

      @@epmanualphysicaltherapy no they said reason i have bone on bone in hip no cartridge left. I dont want replacement surgery if they did that would be better to take leg off ...hip, knee, ankle, foot, along with wrist ....have bone spurs all through body too boot

    Mine is just killing me laying down on my side on my bed! Standing is also hard for longer than a few minutes with spinal issues Stenosis,DDD, arthritis etc…Looking for any kind of relief! It’s been years! 🥲
